SECTION 1
You are advised to spend about half an hour on this section, which contains ONE question.

1 (a) One of the stages of a risk assessment is to identify the hazards.

Identify the hazards associated with crude/unrefined oil during extraction,


storage AND processing. (8)

(b) Identify TWO types of risk assessment techniques. (2)

(c) Outline a typical 3 x 3 matrix that can be used to assess the level of risk. (4)

(d) The summary of risk evaluation is often drawn in a table similar to the one
below.

You are advised to spend about one and a half hours on this section, which contains TEN questions.

2 (a) Identify FOUR properties of hydrogen. (4)

(b) Outline FOUR hazards associated with hydrogen. (4)

3 Liquified petroleum gas (LPG) such as propane and butane is stored under
pressure in spheres.

(a) Outline the typical arrangements to protect the structural integrity of the
sphere. (4)

(b) In the event of an external fire occurring close to the sphere outline the
possible consequences. (4)

4 Identify FOUR types of work activity associated with an oil installation that might
require a permit-to-work AND give a reason in EACH case for the requirement. (8)

5 Within an oil and gas installation effective shift handover can prevent incidents.

(a) Outline FOUR shift handover principles. (4)

(b) Practical operational issues that should be communicated at shift


handover include process problems over previous days, instrument
overrides, supervisor instructions and relaying routine operations for the
next shift.

8 On an oil and gas production platform asset integrity includes testing of safety
critical systems such as fire detector operation.

Outline additional safety critical systems that may be tested. (8)

9 Gases, vapours, mists and dusts can all form explosive atmospheres with air.
Hazardous area classification identifies zoning where special precautions over
ignition sources are needed to prevent fires and explosions.

(a) Identify THREE zone classifications for a mixture of air and a hydrocarbon
gas AND outline the related conditions for EACH zone. (6)

(b) Identify TWO zone classifications for a cloud of combustible dust in air. (2)
