Dear Families,

I am currently working towards my M.Ed. in Applied Studies in Teaching and Learning at West Chester
University. Over the next few months, I will be conducing an action research study in our classroom. The topic
of my research is mathematics problem solving. My goal is to help students develop a deeper understanding
of mathematical concepts. In order to do this, I will focus on providing students with real-world problem
solving opportunities.

I have begun (and will continue to) research the positive impact that various instructional techniques and
learning opportunities can have on students’ abilities. I will now begin applying my learning in our classroom.
Over the next eight weeks, I would like to observe your child as he or she discusses and solves word problems
during our daily math lessons. I want to ensure that the skills that students develop in school can, and will be
applied outside of our classroom walls. We will explore new mathematical concepts through the lense of real-
world situations. I will be collecting student data to inform my work. This will include interviews, student work
samples, observational checklists and notes, and video recordings of students’ solving problems with their
peers. This will not interfere with our regular classroom schedule or activities.

Although every student in the classroom will be engaged in problem-solving opportunities during math class, it
is completely up to you whether or not you would like your student to be a research participant. Both
participants and non-participants will have the same experiences. I do not anticipate any potential risks for
those who participate. If you choose for your child not to participate, I will not interview him or her. I will not
use video recordings and observations of them, nor samples of their work. There will be no impact on your
child’s grade, nor his or her status in school. If you do choose for your child to participate, you may withdraw
your consent at any time, under no penalty.

All of the data that I collect will be kept confidential. My work will include no reference to your child’s name. I
will use pseudonyms. The results of my work will be displayed for an online website. I will be sharing and
discussing my work with my classmates and professor throughout the course. Finally, I will share my findings
at a university-based research conference for M.Ed. students. Again, students’ names and personal
information will not be included.

Please indicate whether or not you would like your child to be a research participant by checking one box in
the space below. Please return one copy of this form to me by Friday, February 9, 2018. You may keep a copy
for your reference. Please do not hesitate to contact me should you have any questions about my research or

 I agree to allow my child to be a research participant. I understand that there is no risk to my child and
that I may withdraw my consent at any time.
 I prefer that my child not be a research participant. I understand that my choice has no impact on my
child’s grades or status in school. He or she will still participate in all learning assignments and
activities.
