RE : Training on ISO 9001:2015

As part of the standardization of legal document of Bureau of Philippine Standards (BPS) and
Department of Trade and Industry (DTI), we have selected qualified employees to attend the
training of ISO 9001:2015.

The training aims to incorporate to our Office the standards for business operation which opens
the opportunity to a higher accreditation (the “ISO”). Consequently, this will provide us ample
knowledge to raise our awareness on how to develop the quality, effectiveness and efficiency
of service to all our stakeholders.

In view of the foregoing, may we respectfully request that a total amount of Nine Thousand
Pesos (Php 9,000.00) be allotted as payment for the registration and other necessary expenses
on the said training.
