IEP Conference Form

Practicum in Professional School Counseling

Counselor:

Attendees: Mother, Student, TVI, special education teacher, special education

director, vocational rehab, school social worker, math teacher, AEA regional director,
school psychologist

Conference Topic or Purpose:

The main purpose of this IEP meeting was to plan ahead for a student with severe
visual needs. This student has had social and emotional issues in the school
building, which has led to the parent homeschooling the student 6 out of 8 period of
the day. Next year, for the student’s final year of high school, the parent and the
student would like to return to EBF Jr/Sr High school as full time as possible. We
also discussed future plans for this student after high school. Regardless of if this
student attends school full time or not, the school is responsible for preparing
materials for the student’s visual impairment.

Critique of Conference:
I truly feel like IEP and parent meetings is one of my strengths as a school
counselor. I have very strong communication skills and am able to keep my thoughts
concise and to the point, while remaining sensitive to issues. One thing I have
known I need to improve on and have acted on that, is to make sure all stakeholders
are on the same page with the important information I share at meetings. When I talk
about graduation requirements and progress of the student, I feel like it can be
almost a foreign language to some people. This is very important information for
members of an IEP team to understand. I have created this tool for everyone to use
and take notes or ask questions with during the meeting. Since I understand it so
well, I tend to probably work too quickly through it. I have received positive feedback
from both special education teachers as well as parents on the use of this tool during
meetings.
