 Distribution of Grades through ¼ size sheet of bondpaper since Form 138-A is not yet
available
 Subject with lacking required outputs
 Araling Panlipunan
 Filipino
 English
 Remediation and make-up activity for those w/ Incomplete grades

2. Lacking Forms

 Photocopy of Birth Certificate

 Promissory letter for unregistered in LCR yet
 Follow-up on the Birth Registration

3. Other Matters

 Requesting parents to monitor and encourage their sons/daughter to wear school

uniform with I.D. and
 to come to school before 7:15 in attending the flag ceremony
