I. Lecture about Venipuncture discussed by Mr. Hero Salem
 Prefer using non dominant hand
 Distal to proximal approach
 Rotate access site; relevant anatomy
 Consider catheter material
 First choice is the periphery
 Legs aren’t to be used except in desperate situations
 If there is a difficulty finding an appropriate site, ask for
help
 Do not forget to prep the skin

HOLLIDAY-SEGAR METHOD
• FORMULA METHOD
– Would give about fluid requirements on a 24-hour basis
• 4-2-1 METHOD
– 24-hour period divided into approximate hourly rates to correlate
with the delivery pumps used since these are designed for an
hourly infusion rate.

WEIGHT IN KILOGRAMS DAILY FLUID REQUIREMENT


1ST or ≤ 10 KILOGRAMS 100 ml/kg/day
2nd 10 KILOGRAMS 50 ml/kg/day
>20 kilograms 20ml/kg/day

4-2-1 METHOD

 Emergency purchasing of medications
 Incomplete entry on the prescription pad for dangerous
drug
 For AM charged medications, please receive the patients
copy of transaction slip and affix your signature

 Suggestion to pharmacy to let the pharmacist


deliver the purchase meds direct to patients/SO.
 POD not answering phone call- pharmacy head will call
the attention of the pharmacist involved

2. Radiology
 Request first before procedure- case to case basis esp.
for special area/STAT.

3. Central Supply Unit


 Before transout, confirm if room is prepared already.
 STAT charging of linen, nurses should get linen at the
CSU, do not wait for them to deliver the linen
 They may deliver after lunch or during their availability

4. Out Patient Dept.


 Reiterate to bring discharge instruction/ baby book
prior to discharge during follow ups
 BCG schedule last Saturday of the month- morning

5. Laboratory
 Blood specimen collection by the NOD at the ER.
INCOMPLETE LABEL. Must include full name and date of
extraction
 Extraction at the ER and yet the patient was already
transferred to his/her room.
 Encode laboratory request-
6. Medical Records
 No ER notes for direct to OR admissions
 No form of direct to OR policy.
 Refer to management to revise ER notes regarding
direct to OR patients
 Consent for admission no affixed signature of some ER
nurses (prevalence has been increase for the past week)

7. Intensive Care Unit
 Gowning of patients transferred from gown.

8. Emergency Room
 ER properties cannot be borrowed (ambubag, oxygen
gauge, intubation set)
 Proper endorsement must be observe when using the
ER phones for calling/updating Doctors.
 Patients for IV reinsertion, make sure that all needed
materials are available.

I. General Ward Concerns
1. Return blood bag to the laboratory immediately.
2. Patients for OR- endorse all due medications as well as all
syringes.
3. Check IV sites at all time- to prevent IV BURN, infiltration,
extravasations,.
4. Initial assessment during rounds PLEASE- “nurse patient
interaction” patients first, contraptions, available
medications.
5. Skin testing- 30 minutes after test check for redness, if
positive. Control by using same diluent without medication
into different syringe.
6. Please utilize diagnostic requests- Do not use prescription
pad.
7. Do not puncture IV bottle- entry of microbes.
8. Hooking of IV fluids from other patients (RC)
