1. SCOPE OF WORK

1.1 INTRODUCTION:

Electro Tharwat is a private investment company for trading and contracting established in 1964 in
Cairo Egypt.

Electro Tharwat is able to offer a comprehensive range of professional services associated with a
number of activities such as project management, procurement, installation and servicing in all
areas of electrical energy, Production, Transmission, Distribution and Utilization including Thermal
Hydro Generation and High Voltage substations

Electro Tharwat completely committed to comply with requirements of international standard


OHSAS 18001.

Electro Tharwat works as a consortium leader for the contractors of Nubaria power station III in
association with (EGYTECH power cables).

The project safety program addresses and complies with all the safety issues raised in this project
contract specification .In addition, the safety program is also fully complying with All applicable
laws, regulations , standards, and codes governing safety and health in the workplace .

Electro Tharwat for Trading & Contracting has developed its Occupational Health and Safety
Management System to comply with the international standard OHSAS 18001

1.2 OBJECTIVE & PURPOSE:

Objectives:

Objective is to have a safe and accident free project execution

Department Managers are responsible for suggesting OH&S objectives taking the results of risk
assessment, technological options, financial and operational requirements and views of the
interested parties into consideration

Purpose of the safety program:

The purpose of the Safety program is to:

- Avoid risk of harm to the health and safety of persons and property and for inspecting and
monitoring all our equipment, materials and work.

- Protect workers, installation belonging to us or to our customers as well.

2. COMMITMENTS:

2.1 OCCUPATIONAL HEALTH AND SAFETY POLICY

Complete compliance to Egyptian and international standards and all legal and other requirements
to avoid damage and accidents.

Provide safety tools and all precautions necessary for emergency and suitable work instructions
and provide training for all employees

The optimum usage for all resources and equipment using the most up dated technology that leads
to continual improvement through monitoring and measurement and internal audit process.

Provide adequate training and awareness for all employees to promote their skills and complete
awareness with OH&S requirements

Identify hazards and bad impacts and put plans and necessary procedures to prevent and mitigate
their consequences and their recurrent in the future.

Put positive objectives and create suitable work environment to enhance performance and review
implementation for objectives periodically.

2.2 WORKER'S WELFARE& LEGAL REQUIREMENTS :

WORKER'S WELFARE:

Worker morale and loyalty are viewed as fundamental to maintaining an accident free work
environment. They are the most valuable assets of the company and Electro Tharwat will exert
every effort in their protection of potential hazards and/or harm while at work place.

Electro Tharwat will ensure that the safety of all workers shall be given top priority and shall
provide Acceptable facilities, PPE, training, information and incentives.

- Electro Tharwat shall provide its employees adequate supply of PPE

and will conform to international standards.


- Electro Tharwat personnel shall undergo the necessary training /seminars

Organized and conducted to familiarize and ensure clear understanding of

the hazards associated with the construction and operational activates and

ways to control the hazards.

- Ensure the provision of common transportation from accommodation to

the respective work place.

B. ACCESS TO THE CONSTRUCTION SITE


Access to the construction site shall take place in accordance with the customer’s rules, in this
sense; agreements may be reached with the site manager. Vehicles may only be allowed onto the
site in exceptional cases and only to a limited extent .The equipment used by the workers must be
shown on the inventory list drawn up by the subcontractor (in duplicate, one for each party)

C. LEAVING THE CONSTRUCTION SITE


Leaving the construction site shall also take place in accordance with the customer’s rules.

Badges, parking slip, etc must always be presented


D. TRANSPORT OF PERSONNEL
The subcontractor shall be responsible for transporting personnel, to and from the site.

The driver must have a valid driver’s license. The vehicles must have a valid inspection certificate.

E. PERSONAL INFORMATION REGARDING WORKERS


The site manager shall be in charge of drawing up a list of the subcontractor’s employees prior to
commencement of the commissioned works. He must be provided with, at the very least, the first
and last name, date of birth, nationality and professional qualifications.

Foreign workers must have a valid passport and work permit according to the Egyptian regulations
and must be able to express themselves in the customer’s language (English)

The minimum age of an employee working for the consortium shall be set at 18years.The
subcontractor must designate a site foreman for each project, who shall represent him and exercise
his authority

3 ACCOUNTABILITY FOR SAFETY :

Occuptional health & safety is a fundamental to the operations of Electro Tharwat and its primary
objective is to have an accident free project execution . it's every one's responsibility to understand
the requirements for working safety and protecting themselves and others .to achieve this goal the
following accountabilities are expressed ,to assist all personnel in understanding their all specific
resonsibilities.

3.1 MANAGEMENT RESPONSIBILITIES:

- site project manager & construction manager shall take the full Management responsibility for
the project site OH&S performance according to Electro Tharwat policy and procdures.

- Ensure that a full risk assessment is undertaken for all activities, provide writtien instructions to
establish safe working methods .

- Follow up and expedite any encountered problems to the OH & S officer.

- Attend the coordination and safety meeting help in providing safety resources to facilitate the
implementation of safety plans.

-ensuring the implementation of safety plan.

- ensuring the training and awareness of working personnel for all safety issues .

3.2 RESPONSIBILITIES OF LINE SUPERVISORS/FOREMAN:

Supervisors and foremen are directly responsible for the implementation of the OH & S policy and
to ensure all their personnel understand and comply with OH&S policy and procedures that relate
to their job activities.
- Supervisors and formen are directly responsible for maintaining safe

working conditions ,utilization of safe parctices, the safety of personnel

under their supervision and the safety of others affected by the

activities of their personnel.

- Participate in the investigation of all incidents, to ensure timely action

shall be taken to prevent recurrence .

- Provide continuous site inspection in this area of responsibility to identify

and correct any unsafe conditions including the assurance of providing

(PPE) personal protective equipment to their work men.

3.3 RESPONSIBILITIES OF HSE PERSONNEL AND SUPERVISION:

Electro Tharwat shall appoint a responsible HSE officer and HSE supervisors who shall be
responsible for ensuring and monitoring that the work is performed in accordance with the
applicable OH&S requirements.

- Electro Tharwat HSE personnel will work closely and follow up all

matters pertaining to safety , including the implementation of OH&S

guidelines that will be issued from time to time as the work progresses

and determining the appropriate level of coverage.

- electro tharwat is committed to distribution OH & S suppervisor adequate to cover the


suppervision of site working personnel .

- Electro Tharwat HSE personnel will participate in job assessment to

ensure all precautions are being taken to ensure the safety and well-being

of employees is protected and to eliminate personnel injury and damage to property,and


encourage the participation of labour force representatives.

- All personnel are authorized to use any alarms means or air horns being

utilized at site in the event of alarm or obvious hazardous condition .

HSE officer personnel to ensure that such provisions of portable air horns

are available within the vicinity.


3.4 SUBCONTRACTOR OBLIGATIONS (IF EXISTING)

The act of submitting a bid shall imply that the Subcontractor is able to:

- Adhere to and apply the safety conditions.

- Legislation and social provision.

- The subcontractor furthermore declares that he has visited the site where

the commissioned works are to be carried out and that he is aware of all

indispensable information. In other words, the subcontractor declares

that he has informed himself of the risks and specific nature of the

commissioned works.

- When drawing up his bid and determining the price, the subcontractor

shall take adequate account of the safety information presented or made

available to him beforehand.

- The subcontractor shall therefore refrain from claiming additional

compensation from Electro Tharwat in and above his pre-determined

price.

4. TRAINING ,OH&S MEETINGS,TOOLBOX TALKS & AUDIT:

4.1 TRAINING:

Instructions and safety training of all the Electro Tharwat and its subcontractor employees will be
prepared, organized and explained by the safety officer of each subcontractor attended of necessary

with other people such as Doctor, Local fire Brigade, etc.

Before beginning works, on the site, an attendance form will be established and registered, filled
with the employees' names, the dates and subject of the training.

A one-day safety meeting is conducted and recorded.

4.2 OCCUPATIONAL HEALTH & SAFETY MEETINGS

Prior to commencement of the commissioned works, a preliminary meeting shall be held with the
participation of:

- The site manager


- The site supervisor

- The safety officer

- The safety department (if needed)

- Any concerned parties

This meeting shall cover the following items:

* Layout of the site

* Safety aspects relating to the commissioned works

* Discussion of additional safety instruction (namely, the prescription

given by the customer)

* Scheduling of a meeting to inform all of the workers assigned to carry

out the commissioned works.

* Passes to gain entry to the site

* Scheduling toolbox meetings and drawings up attendance sheets.

* Determination of later site meetings and participants.

* Discussions on:

- Provisions regarding individual protection gear

- Provisions regarding language to be used

- Provisions regarding the selection of workers and specific certificates

Training in the area of safety, hygiene and environment, for specific

jobs.

Weekly supervisor safety & health meeting.

Weekly Toolbox safety meetings will be attended.

4.3 INCENTIVE SCHEME:

Electro Tharwat has developed HSE incentive scheme to promote awareness, knowledge,
commitment, responsibility, and safety consciousness amongst employee.

4.4 TOOL BOX TALKS:

- It is mandatory for all supervisors/foremen to conduct and deliver tool


box talk at work locations within their respective area prior to

commencement of daily work assignments.

- Supervisors /foremen shall ensure that potential hazards of the tasks and

specific safety requirements required when executing the tasks are clearly

understood and briefed to all the personnel involved and ensure that safe

working procedures are strictly followed.

- Emloyees assigned for specific works shall sign the attendance sheet after

completion of tool box and this shall be attached with the work permit(if

applicaple).

- Topics to be discussed shall include as applicable the following.

Emergency procedures

Muster points/alarms

PPE

Site safety regulations

Houskeeping

Fire prevention

Safe working practices

Emolyees safety responsibility

Eye protection

Tools

Short cuts

Confined space safety

Near miss

4.5 HSE SITE INSPECTION /PERFORMANCE SITE MONITORING/AUDIT


PROGRAM /SCHEDULE:

- Electro Tharwat deputy HSE shall establish an OH&S audit programme

HSE performance site monitoring, the findings shall be forwarded to the Electro Tharwat OHSE
manager and project manager .

- Electro Tharwat project HSE & project manager shall

follow –up all outstanding items .

- Corrective actions shall be implemented. Copy of OH&S audit /HSE

performane site monitoring report shall be retained by the ETW OHSE

department.

- Findings of the daily HSE officer's inspections are logged in individual

field books and daily inspection sheets . HSE supervisor check daily and

ensure that all deficiencies that have been noted are corrected /rectified

promptly. HSE officer shall ensure like wise that appropriate

rectification is constructively done in coordination with the project

management team .

- Weekly OH &S inspection /walk around shall be organized by Electro

Tharwat HSE department.

5. SPECIFIC REQUIREMENTS:

5.1 PERMIT TO WORK (PTW) IF REQUIRED /APPLICABLE:


Electro Tharwat personnel assigned to supervise work that is required to be executed under
permits shall attend a permit to work (PTW) system training courses conducted and organized by
Electro Tharwat only those persons trained and certified in accordance with (PTW) system may
supervise workers that are to be permitted.

Objective:

The objective of the PTW is to ensure that written permission and autorization is given to carry
out the defined works that are potentially hazardous and that all possible measures are taken to
maintain the safety of the personnel and equipment that may be affected by the works.All staff and
personnel must fully comply with PTW requirements.

Purpose

The purpose of these regulations is to set down a system for the safe control and smooth operation
of work which:

- Provide for maximum safety of personnel and plant /substation works.

- Provide for good co-ordination between plant operation, maintance and

the contractor /subcontractor.

Function

- Describes the nature and extent of the work to be conducted.

- Describes the condition that have been observed and planned.

- Records the exact location in which men other than the operating

personnel will be working for a specific time.

- Gives permission for works to be done .

- Details any remaining hazards and precaution to be taken.

- Ensures co-ordination between concened parties(Engineer-Consortium- Owner).

Rules of work permit:

No work permit – No entry No work

One permit - One receiver

5.2-WORKS INSIDE CONFINED SPACE

Works inside confined spaces as gutters, tanks, tubes, exchangers, boilers, condensers can start
only after issuance of a work permit and completion of the following specific measures:

-Ensure good access,


-Check that dangerous or suffocating products do not remain,

-Check that ventilation and ambient temperature are acceptable,

-Check that parts under fluid are tights as well as the absence of pressure,

-Fluid valves are to be locked by padlock,

-Products not to be used inside confined spaces are to be listed and their use forbidden

-Permanent worker outside ensures surveillance and availability of first aid measures,

-Lighting and electrical tools shall be supplied in low voltage (24V)

The operator shall be provided by safety belts connected to a rope of suitable length ,both if he is
assisted by another worker working and giving him help, if necessary, in case of illness and /or
injury .

5.3 PERSONAL PROTECTIVE EQUIPMENT (PPE):

Electro Tharwat shall supply its staff and emloyess with PPE meeting designated standards.

This shall comprise of a minimum ;safety shoes overalls ,hardhat and safety glasses .these items are
to be worn at all times while in the

plant /substation or outside of office accommodation.Welders may be excluded from wearing of


hardhat while using welding face shield. however ,personel working at heights 2 meters and above
shall use a full body harness with double landyard to be tied off at all times .

PPE Shall be maintained in good condition ,any damage due to natural reason a replacement shall
be issued.

5-4 WASTE MANAGEMENT / ENVIRONMENTAL PLANS:

Electro Tharwat shall comply with and ensure that all Environmental requirements stipulated by
the contract for disposal ,storage, handling of hazardous and non-hazardous materials shall be
strictly adhered to Electro Tharwat shall ensure that the disposal of hazardous and non-hazardous
materials utilized on site is in accordance with Egyptian governement regulations.

5-5 SCAFFOLD /LADDER & ACCESS REQUIREMENTS :

Electro Tharwat shall ensure that all necessary&essential requirements with regards to Scaffold /
Ladder & Access Requirements. Execution of work shall be in compliance to Electro Tharwat
procedure HSE, Scaffolding Procedure, Contract Scaffold Requirements & other safety standards.
Electro Tharwat personnel and /or approved subcontractors will carry out scaffolding and provide
access requirements for Project. materials to be used during the construction shall be inspected
and/or approved by material controller prior to mobilization on site.

Qualifications / Certification of scaffolders will be submitted to the Engineer for information &
approval.

Scaffolding must not be used until approval has been given and a green tag has been placed.

The condition of the scaffold will be entered in a weekly inspection records and shall be available
for audit at all times.

5.6-FALL PREVENTION &PROTECTION

Scaffolds, ramps, ladders, and platforms shall be provided for all works to be done 2meters above
the ground.

-Unauthorized persons shall be prohibited from entering the construction area scaffoldings, ramps,
and platforms.

The type of scaffolding shall be in accordance with Egyptian regulation All scaffolding can be
erected or modified only by skilled personnel/ authorized scaffold specialists following the
recommendations of the manufacturer manual and can be used only after checking and approved
by a competent person, and this approval will be clearly indicated

-ALL scaffolding should stand on a solid, stable and firmly compacted soil and tied with fixed
support.

-Ladders must be properly fixed, with no slip rungs and protective cage where appropriate.

-Incline Ladders correctly and always face the ladder when climbing up getting down off the ladder

-All the ladders must be inspected before and after use and any deficiency observed shall be
corrected immediately

Any ladder spit, damaged or missing rings must be destroyed and not used

-Home made ladders are not to be used

-Stairways must have non- –slip steps and handrails

-Personnel elevators must be properly maintained and regularly checked.

-Footbridges across trenches are to be provided with handrails

-All accesses to height, event temporary, must be Kept clear and designed to allow emergency
access for first aid, evacuation and fire fighting,
-It is forbidden to remove handrails, gratings without the provision of temporary closing plates or
personnel protection barriers.

Works at height must be protected, where possible, by handrails and toe boards

. –Scaffolds, ramps and platforms shall be kept free from grease, mud or any other material or
equipment.

Any works at a height over 2 meters without a safety belt and safety net shall be prohibited.

If necessary nets must be installed to prevent the fall of materials into other working areas

-All the opening must be closed permanently.

-Seal the openings with standard fencing or correctly fitted cover plates.

-Install standard fencing to warn of all unsafe areas.

-Install catwalks and check that they are not obstructed.

-Install standard fencing to warn of all unsafe areas

-Any safety device or protection will never be removed without approval from the safety officer

Check once a week that all these measures are respected

5-7 DRIVING SAFETY AND TRANSPORTATION :

Transport and lifting activities often cause a relatively high number of industrial accidents.

- This section outlines the principal hazards and the precautions to be taken in the transport and
lifting of materials.

a- In accordance with the provision, all vehicles authorized to enter the site area shall respect the
following conditions:

- Motorized equipment shall be operated only by a licensed driver.

- Drivers may use only fixed or temporary roads

- Drivers shall respect all signs.

- Drivers shall ensure the stability of their load during transportation

and when stationary

- Vehicles shall be parked only in designated parking areas.

- Speed limit: maximum speed is 15 KM/h, at the site.

- Use of vehicle horn is prohibited except in case of immediate danger

- All persons engaged in construction shall use the designated


Entrances only and should not walk through or around the existing

plant at any time

b- Any violation of these rules will result in an immediate banning of the driver from the site.

5-8 CRANE AND LIFTING EQUIPMENT:

- Transport and lifting activities require proper training of personnel

involved.

- Supervisors will keep alert cases of mal operation, due to possible defects

of the plants.

- Workers performing lifting operations will adhere to established safe

procedures and will use only certified equipment in good conditions.

Forklift Trucks

- The driver must make a daily inspection of controls, moving parts and

wheels.

- Defective brakes, controls, lights, battery, lifting system, steering

mechanisms or signaling equipment must be repaired before further use

of the truck.

- Important precautions for truck operators must be taken:

* Never use a forklift truck for any purpose other than that, for which it

has been designed (in particular, never use it as elevator for workers).

* Never attempt to lift a load greater than the maximum load permitted

for truck.

* Never move loads which are insecure.

* Never allows anyone to stand or work under elevated forks, whether

they are loaded or not.

* Avoid making fast starts, jerky stops or quick turns, particularly if

materials are being stacked at a height.

* Always move the truck in reverse, if the view in the forward direction
is limited by the load.

* Slow down, and stop, if necessary, at blind corners doors, entrances,

etc., and let another worker lead you.

* Check that overhead clearance is sufficient for maneuvers.

* Always travels with works, as low as possible, i.e.10-15cm above

ground,

* Avoid causing accumulation of fumes and gases by long periods of

idling petrol or powered tucks in enclosed or semi- enclosed areas

5-9 EQUIPMENT CERTIFICATION :

Electro Tharwat shall ensure that all Electro Tharwat and subcontractor (if existing) lifting
equipement and machines have been inspected and certified by an approved authorized agency.
Certificates shall be available to Engineer review.

5-10 CRITICAL ACTIVITY PLANNING :

Electro Tharwat Shall carry out JHA ( Job Hazard Analysis ) for critical tasks to identify hazards and
controls required.

All critical activity shall form part of work permit conditions

5-11 ELECTRICAL ACTIVITIES.

5.11.1-ELECTRICAL WORKS

The use of electricity presents two main hazards: direct injuries due to electric shock and /or burns,
and as source of ignition, which could result in fire or explosion Only skilled and authorized
electricians shall be allowed to work on electrical installations They shall work according to
professional and statutory safety rules. Electrical installations shall comply with Egyptian safety
standards

Common requirements are summarized as follows:

Electrical equipment are to be firmly connected to the earth,

Electrical supply cables are to be protected and marked,

Electrical boxes are to be solid, locked by keys and connected to earth,

Electrical circuits are to be provided with an instant earth leakage system and appropriate fuses,
Electrical systems and equipment are to be provided with emergency cut off devices.

-Electrical works: equipment shall be locked or secured to prevent starting by unauthorized


persons.

Electrical supply and equipment used in confined or wet spaces shall be “low voltage” with well-
isolated or remote transformers.

Electrical receptacles and outlets to be provided with ground fault circuit interruptors (GFI).

5.11.2-ELECTRICAL NETWORKS

General precautions:

All conductors and equipment should be considered live unless

There is a certain proof to the contrary

The site is traversed by various cables or pipelines, which are laid underground.

Any cable or pipeline damaged during works shall be reported to the authorized representative of
the owner.

The crossing of a road by high –voltage line wires is boxed in a frame –sized protection.

The location of all buried networks shall be recorded exactly before backfiring.

Electric cables shall be tagged by an ordinary mechanical protection grid placed 0.2m above and by
red surface signs indicating the route

5-12,13 MATERIAL HANDLING AND HAND TOOL

All employees working on the job site shall wear, in a well visible way, a company stick at the front
of the helmet and /or a badge on their outer garment indicating their name, registered number, at
all times within the job site.

All workers shall wear suitable working clothes:

- Helmet and suitable safety shoes: compulsory in all working areas.

- Ear protection: compulsory in areas of loud activities (above 90 dbA).

- Safety goggles: compulsory for actives dangerous to the eyes

(Ex. grinding)

- Welding mask: compulsory for welding and grinding

- Harnesses: compulsory for works at height, in the absence of any

proper safety railing.


- Full visor goggles and gloves: compulsory for work with hazardous

fluids (chemical, gases under pressure)

- Gloves: compulsory for activities may burn or harm hands before the

job commences, the Foremen of the team will check that precautions

taken are adequate and, if necessary, they will suggest improvements.

- Barriers and signal for dangerous areas:

Excavations, holes trenches, electrical work areas or any hazardous area must be surrounded by
appropriate barriers or signaling strips according to the nature of the danger. Once the danger is
over, barriers or marking should be removed at once. When necessary, the designation of the

dangerous area may be completed by a warning light.

Protection against noise:

Exposure of personnel to noise levels over 85 DB (A) can result in permanent impairment of
hearing, especially noise values over 90 DB(A). Correctly fitted ear protectors must be worn
whenever personnel are exposed to the aforesaid noise levels, such as near large machinery,
particularly in enclosed areas, when using compressed air grinding wheels, pneumatic hammers,
etc.

All air compressors for air supply or electrical generators will be soundproofed.

5-14 SITE HOUSEKEEPING, SANITATION & HYGIENE:

- All construction areas, waiting rooms, offices, workshops, and storage

yards shall be daily cleaned.

- All garbage, rubbish and papers and saw dust shall be daily cleared from

work sites and taken to the designated dump area regularly.

- All metal /wood scraps, insulation rubbish, empty paint cans, etc. shall

be properly Collected (differentiated waste collection) and placed at the

designated location.

- Under no circumstances, shall any waste materials be dumped into the

lake at any time.

Hygiene:
- Offices: sufficient lighting, good ventilation and a frequent cleaning are

to be ensured.

- Sanitary facilities: shall be placed in sufficient quantity and regularly

cleaned

- First aid: at the beginning of site activities, first aid materials shall be

provided for treatment of minor accidents and emergency interventions

for incidents which may arise during the site construction activities,

- Each party will conduct a survey to establish the total facilities available

for emergency care in case of accidents and incidents and a procedure

prepared for their personnel evacuation.

- Cleanliness: regular cleaning of working areas and premises to ensure

hygiene and access clear of obstruction.

5-15 TAG OUT / LOCK OUT :

Tag Out and Lock Out procedures shall be followed when applicable

Tags:

DANGER- DO NOT OPERATE tags are written in English, ensure everyone understands

Tags shall be affixed to any equipment, tools, or sub-standard items, which are not to be used or
operated

DANGER- DO NOT OPERATE tags shall be affixed and removed by the responsible / authorized
persons after a through check has been made to ensure it is safe to do so.

Locks:

The application of a Padlock or Scissors Lock makes it impossible to throw a switch or lever to ON
position

Each electrical isolation switch, which is equipped with a mechanical stop, shall be locked in the
OFF position with a padlock or Scissors lock

5- 16 WEATHER (SANDSTORMS / EXTREME)

In the event of dust or sand storm, movement of vehicles and personnel on site is restricted to
essential task. If the weather becomes extreme that restricts the visibility of the personnel
temporary termination of work may be decided by the management / site supervision
5-17 HEAT

Heating devices such as coffee pots and /or cooling equipment shall be maintained in a safe
manner.

- No cooking equipment is allowed to be used at any time.

Symptoms of Heat Stress and First Aid:

Heat illness comprises wide spectrum or problems ranging from minor inconvenience to critical
emergencies and death. Subject to heat should be avoidable by a combination of knowledge and
good management

5- 18 SUBSTANCE ABUSE PROGRAM:

Electro Tharwat shall ensure all of its employees & subcontractors personnel working within the
project follow & comply with the substance abuse Policy & other applicable local regulations set
forth by the ministry of Electricity and Energy if there is any regulations.

6.0 ACCIDENT / INCIDENT AND REPORTING & EMERGENCY


PREPAREDNESS

6.1 ACCIDENT / INCIDENT AND NEAR MISS REPORTING

Reporting shall be in accordance with Electro Tharwat procedure Accident and Incident Reporting
Procedure unless it is required for Electro Tharwat to adopt any other Incident / Accident and
Investigation Reporting system.

All Accident and Incident near misses and spills shall be recorded, reported to Engineer
immediately and investigated; Generated reports must be submitted within 24 hrs.

For the purpose of accident reporting and investigation, an accident is defined as an Unexpected,
unplanned undesired event that result in

Physical harm (injury or disease) to individual,

Damage to property,

Near miss,

Any combination of the above.

In the event of personal injury to any of Electro Tharwat personnel, or in the event of any loss or
damage to the work, materials, supplies or equipment supplied by either the owner or Electro
Tharwat for incorporation in to the work, or any third party equipment, or an incident that results
in fire, leakage of hazardous or toxic materials, or any near “near miss” situation which might have
resulted in personal injury to any employee of Electro Tharwat or any third party. Electro Tharwat
shall immediately notify Engineer Representative and complete and submit the Accident / Incident
Report. Notwithstanding the aforementioned the following actions are to be carried out by Electro
Tharwat.

In the case of a fatal or major accident, notification shall be forwarded immediately to the
concerned authorities through Engineer / Electro Tharwat authorized personal.

The WORKSITE shall be secured in the undisturbed state as soon as is safe to do so.

Names of witness and any evidence shall be secured. A preliminary investigation shall be initially
done and the Accident Report completed within 24 hours.

The investigation shall aim at:

Establishing the events leading up the incident.

Establishing the fact of the incident itself.

Establishing the relevant fact of what occurred immediately after the incident.

Identifying the required action to be taken in order to prevent a similar occurrence in the future.

Identity the causes of the accident (Note: causes could be either unsafe conditions or could also be
due to either direct cause on contributory factors).

Any Accident Report involving injury to Electro Tharwat personnel shall be followed up with a
written statement from Electro Tharwat giving consequences / injury such as:

First Aid, Medical treatment given (where, number of days), disability, etc. Initially it should
be clear in the Accident Report whether it is a

first Aid or lost time accident, and with the follow up report statement

as soon as the consequences are known.

6.2 FIRE PREVENTION, PROTECTION AND FIGHTING

- A fire protection plan will be issued and submitted for approval to

the authorized Representative of the

Contractor/Owner.

- Open fire shall be prohibited unless prior approval has been given by the authorized
representative.

- All access to fire fighting equipment shall be kept clear at any time.

- fire extinguishers shall be used in case of fire only.


- A Common alarm, fire fighting and evacuation procedure shall be set up

between all parties, based on the facilities available within the site vicinity. (see appendix 6)

Smoking

Smoking will be allowed only in the designated area under controlled conditions.

Smoking is prohibited in vehicles at the job site.

Fire Extinguishers:

Electro Tharwat shall provide an adequate number of fire extinguishers and sand buckets to cover
the risks induced by its activities and the surrounding dangers.

The amount and location of fire equipments will be defined and agreed with the authorized
representative of Engineer.

Electro Tharwat shall train enough personnel for a proper use of those equipments.

6.3. SECURITY:

Site guarding:

- Movement and presence of personnel shall be limited to their

working area.

- The security shall be assured by Electro Tharwat for its own activities

and its sub-contractors activities if there are any.

Site access:

- Only authorized personnel and vehicles shall be allowed to enter the

site area.

- The following details are required in due time so that provisions for

permit issue can be assured:

List of vehicles for which site access is required giving the type, registration and function.

Name of personnel and its function.

Particulars of any deliveries to site in accordance with the agreed procedure, with specific advance
notification for heavy or abnormal loads.
Details of materials leaving the site. In all cases, materials shall only be allowed to be removed from
the site with authorization of an authorized representative of the Owner

6.4. EMERGENCY PROCEDURE:

In case of emergency:

- All work shall-stop at once.

All equipments shall be shut down and electrical switchboards shall be isolated.

All personnel shall be evacuated to a safe place; a roll call shall be taken to determine if any worker
is missing.

In case of any missing workers, the emergency squad in charge of the concerned area shall try to
locate them immediately,

No one shall be permitted to return to work until authorized safety personnel have designated all
areas as safe.

The authorized representative of the Contractor/Owner shall be informed immediately about all
emergency situations such as serious personnel injury, fire and critical damage to equipment.

Electro Tharwat shall give the authorized representative of the Contractor/Owner the name and
location of its representative to be contacted outside working hours in case of emergency involving
its working area.

6.6 FIRST AID FACILITIES

First Aid is also available in Safety Training Center at Electro Tharwat Head Office.

A First Aid Box shall be available at Electro Tharwat Safety officer

A First Aid box will also be available on site.

first aid well trained personnel will be available during working hours.

List of doctors and hospitals will be available on site.


Emergency phone numbers and an agreement with hospital will be provided.
