Terms of Agreement
For sponsors, please return this agreement by April 15th to: CFSD Foundation, PO Box 65150, Tucson, AZ, 85728-5150 or via
email to toy@cfsdfoundation.org. Payment for sponsorships, tributes and advertisements can be returned with this form or made
online at cfsdfoundation.org. It is the responsibility of the purchaser to send logo and/or advertisement via email by April 15,
2018 to toy@cfsdfoundation.org. Acceptable file formats include jpg, png, gif and tiff at 300 dpi and saved as High Quality.

CFSD Foundation agrees to provide benefits as listed above for the appropriate advertisement or sponsor level. Sponsor agrees to
pay amount indicated. The parties acknowledge that they will use their best good faith efforts to negotiate and resolve any issues
that may arise from this Agreement as a result of unforeseen occurrences.
