Description of how to implement in the class:
Google forms are a handy tool to do instant student assessment. This task can be completed on either a
computer, Ipad or smartphone; in the classroom or from home. Students can either follow a link or
scan the QR code above to access the quiz I created. Once they access the quiz they can go trough and
answer all the questions at their convenience. As they finish they can submit the quiz and based on the
need at the time, they can receive instant feedback or I can go back and evaluate the answers and
direct my classroom as needed.
Level of Technology Implementation (LoTi):
These quizzing method used by Google forms best fits in LoTi level 2 which is exploration. The google
forms are based on content understanding that focuses on comprehension.
Description of the sample spreadsheet and graph/chart provided *:
Below is an example of what a chart looks like for a problem. I had 13 people take my online
polynomial quiz. The answer for this specific was indeed c, so based on the graph we can easily see
that majority of my students did answer this question correct. Since there most of the students were
on the right track I could easily go and work with the two students who answered this question
incorrectly and help them instead of reviewing this with the entire class.

Another useful benefit I found using this tool was based on the last question of my Google Form. It
allowed the people who took the quiz to let me know what they felt they needed more help on. This is
a very useful tool for students to tell me what you need help with, instantly.
