- Xerox copy of graduation / post graduation mark sheet / certificate duly attested
- Relieving letter of previous employer’s if any.
- Medical fitness certificate duly issued by the doctor (MBBS OR ABOVE)
- Attested copy of driving license.
- Xerox copy proof of residential address duly attested.
- Company’s employment bio-data form duly filled and signed by you.
- First DFAR duly signed by you and by your manager.
- Induction report duly signed by you and by your manager.
- PF/PPF form 2 declaration & nomination ….form enclosed
- Gratuity from/nomination form/form enclosed.

Please note that delay in the submission of the documents, will result into withholding
salary and TE bills. You are therefore requested to send the required documents by return post
for further needful at our end.
