Please have driver call dispatch at 714-734-5700 to report any problems picking up or delivering on time as soon as
possible. Load must deliver to scheduled delivery times. There will be a $150.00 fine for missed pick up and
delivery. Any issues regarding after hours delivery or detention, driver or carrier must call 714-734-5700
(24Hrs/Day, 7 Days a week) and notify Logistics America of the situation, at the time of occurrence. NO
ADDITIONAL CHARGES WILL BE PAID TO THE CARRIER UNLESS LOGISTICS AMERICA IS FIRST
NOTIFIED. LATE DELIVERIES MAY COST CARRIER UPTO $100 PER DAY.

Directions supplied by Logistics America or its Customer’s either orally and/or in written form are for informational
purposes only. It is the carrier’s sole responsibility to confirm that it may lawfully operate a loaded vehicle of any
weight, commodity or dimension over any road, highway, bridge or route. Carrier shall be solely responsible for
any fines, penalties or citations occurring as a result of operating any vehicle over any road, highway, bridge, or
route in violation of any regulation, law or ordinance. Carrier must advise Logistics America of any and all
accessorial charges or other charges in excess of and/or in addition to the agreed rate set forth in this or any Rate
Confirmation Sheet within 24 hours after delivery of each shipment. Failure to provide such information could
result in the failure of Logistics America to collect for such charges, and therefore, payment of any such accessorial
charges or additional charges shall be at the sole discretion of Logistics America.
