Incompetent Operators 2 5 10  Lifting equipment shall be operated only by authorised personnel 1 5 5
who must be properly trained, competent and fully conversant with
all aspects of safe operation of lifting equipment, and be familiar
with the controls and capabilities of the equipment they operate. A
current certificate of competence is required.

Persons being trapped by the 2 5 10  An unobstructed passageway of not less than 600 mm wide to be 1 5 5
slewing motion of the crane maintained available between the counterweight and any wall, other
equipment etc.
 Where this cannot be achieved hard barriers are to be erected.

Crane operator unsighted by 2 5 10  Banksmen shall be appointed where it is necessary to ensure that 1 5 5
buildings / obstructions clear and precise commands are given to the operator, either during
lifting operations or when the crane is being moved.
Banksmen shall be:
 Thoroughly trained in a hand signalling system (BS 7121)
 Capable of giving clear and distinct commands over radio .
 Capable of directing the safe movement of equipment and its load to
maintain the safety of all equipment.
 Identified by orange HI-Visibility vests.

Severity (S): 1-minor 1st aid, 2-Minor hospital referral, 3-RIDDOR (over 3 day), 4-Major injury, 5-fatality
Likelihood (L): 0 - Very Remote, 1 - Remote, 2 - Occasional, 3- Probable, 4 – Frequent
Hazard Rating S x L (0-3 = Low, work can 4-7 = Medium, Work needs additional controls, 8 or more = High, Work shall not commence)
