A. Family History

There are 8 members of her family, her mother, father,4 brothers, sister and
herself. They are close to each other. Siblings are apparently healthy. The father and
mother of the patient was married. Both parents are not too strict. They are
compatible with each other, And She stated that they’re financially stable at the
moment.

B. Personal History

1. Prenatal history and mother’s pregnancy and delivery patient was born
term via NSD.The length of pregnancy of the patient was around 37-38
weeks. There were no known birth trauma or defects. The client stated
that she was planned and wanted as well.
2. Birth
The patient was born on October 14,1996 at Jose Reyes Memorial
Medical Center. Her parents and siblings were happy meanwhile her 1st
brother was 6 years old and her 2nd brother was 4 years old at that time.

3. Infancy
The patient was breastfed and bottlefeed by the mother. There were no
known feeding problems, weaning problems. No problem in sleeping. She
also stated no physical or sexual abuse happened. The infant was playing
with her siblings

4. Early childhood/pre-school
All family member were closed to each other. There was a normal
language and motor development and proper toilet training. There were
a number of playmates she had when she was a child including his
siblings.She remembered “bahay bahayan” as her favorite game. No
physical, emotional or sexual abuse happened.

5. Childhood
a. School
She started to go to school at the age of 5 years old for nursery school
and eventually went to kindergarten. Patient performed average in
 school. according to her,elementary life is the most
enjoyable life she have ever experienced,and it’s the best.

She said that High School is way too different than elementary.Many
things had changed.As she have experienced in their class,It was bonded with crazy
things,and funny moments. She was a bit of outgoing when she was in high school with a
circle of friends.

b. Health status
There were no illnesses occurred to the patient except cough and
colds from time to time. No accidents or operations that has also
occurred. No allergy to food and drugs.

6. Adolescence
 Peer relationships
-patient said there were around 15 friends that were very
close to her.She has no idea about her social
popularity.She was never engaged to gang activities.
 School history and Cognitive and motor development
-The client is currently at his 3rd year of Nursing school in
Saint Louis University. She said that she was well adjusted
to every school she was into, and she has a neutral
relationship to the teachers. Her favorite studies were
mostly math, and history. The client was involved in sports
such as swimming and badminton. Currently, there were
no intellectual nor motor skills dysfunction or any learning
disabilities.
 Particular adolescent emotional or physical problems
-The client stated slight occurrences of nightmares, and
she has no phobias.SHe has no history of drug use.
 Interests and hobbies

- Playing badminton as one of the hobbies she has the most

interest in. she is involved in swimming in her childhood and
badminton as well.
