 Approved Membership Status Verification Slip
 Proof of Income for Business
-Income Tax Return (ITR) with Official Receipt of Tax Payments
- Audited Financial Statements
-DTI Registration
-Mayor’s Permit

For Micro-Entrepreneurs/Transport
 Sari-sari Store/Mini Groceries, Sub-Contractor, On call Maintenance, Technician –
Brgy.Certificate/Clearance + Properly Accomplished Certificate of Engagement duly
notarized
 Commission Basis – Certificate of Employment w/ company logo + 1 year check
voucher or Income Tax Return (ITR)
 Drivers – Franchise, OR/CR and Deed of Sale + Properly Accomplished Certificate of
Engagement duly notarized

Additional requirements
 1x1 picture (2 copies)
 2 valid ID’s
 Original Copy of Certificate of Attendance during Loan Counseling
 Photocopy of P1,000.00 Reservation Fee Receipt
 Photocopy of 1 valid ID of the signatory in the Certificate of Engagement
 Photocopy of Billing Address under the name of applicant OR; if not, Barangay
Certificate/Clearance
 1 Long Folder
For married applicants
 Photocopy of marriage contract
 Photocopy of 1 valid ID of Spouse with 3 specimen signature
 If the spouse is outside the Philippines – Special Power of Attorney (SPA) notarized prior
to the date of departure or duly certified and authenticated by the Philippine Embassy or
Consulate in the country where the applicant is staying

- Meralco Bill - Bank Statement

- Water Bill - Credit Card Bill
- Telephone/ Cellphone Bill - Cable Monthly Bill

Important Reminders:
Loan Counseling is scheduled every Monday to Friday at 10:30am and 1:30pm
Make sure that your Pag-IBIG monthly contribution is updated.
If with existing Multi-Purpose Loan (MPL) or Calamity Loan, make sure to update your
payments prior to submission of your housing loan requirements.
