Communication During Patient Hand-Overs

This conversation takes place in the hospital, when the leader of team A Cempaka
ward (Wahyu) order a nurse (Nurill) to deliver a patient to the Operating Theater for surgery.
All people in the ward just finished their preconference.
Wahyu : “Nurse Nurill today we have 9 patients to deliver to the operating theater.
Nurse Adi holds for three patients, and so does Nurse Romi. So could you
please holds the others?”
Nurill :”Yes, sure. I think I will deliver Mrs. Setya for first. I will check her preparation
for the surgery.”
Wahyu :”Ok. Keep the spirit among us!”
Nurse Nurill come to Mrs. Setya’s bed in room 3. And check her readiness for her
surgery. Mrs. Setya just laying on the bed.
Nurill :”Good morning Mrs. Setya, I’m Nurse Nurill. This time is your turn to go to the
operating theataer for your appendictomy. Are you readu for that? I’m sure,
you have prepared youself.”
Setya :”Yes, Nurse Nurill, I’m ready for the surgery. You will accompany me?”
Nurill :”Absolutely yes. Let us go to the operating theater. But where is your
husband?”
Setya :”He is waiting for me in front of the operating theater. With my son and my
brother.”
Nurill :”Ok, let’s go.”
In the operathing theater...
Nurill :”Mrs. Setya, we have arrived here. Wait for a moment beacuse I have to do
patient hand-over with the surgeon nurse.”
Setya :”Ok, Miss.”
Nurse Nurill walks to the nurse station and have a conversation with the surgeon nurse
(Nurilla). They’re doing a patient hand-over.
Nurilla :”Hi Nurse Nurill. Good to see you today. What can I do for you?”
Nurill :”Helo Nurse Nuril, I just deliver my patient. She will have an Appendictomy. I
need to talk to you about her safety issue, the quality issue, and you have to
know about her status, her treatment, and the protocol.”
Nurilla :”Ok, I will pay attention to it. Let us come up to her.”
Nurill :” Mrs. Setya, this is Nurse Nurilla. Well, today is Thursday October 24th 2017
and this is Mrs. Setya. She was 23 years old with Appendicitis as well as
intestine perforation. She was admitted on Monday, October 23th 2017 at
08.30 a.m. from policlinic because she felt a severe pain and she is not planned
yet for her disgharge by Doctor Stella. She say just look for her conditions post
appendictomy. I have given her antibiotics at 07.30 a.m. and she has done her
fast since yesterday evening. Her medication is Cefazolin 1mg per 12 hours,
Ketorolac 1ml per 8 hours, extra Ranitidin and Paracetamol tablet if she has
high tempetaure. The DC chateter has set. She is monitored by Doctor Stella
and she will doing her Appendictomy. And for sure, I have shear her belly. Once
again, please prepare for oxygen therapy. Last night, she got a little dyspneu.”
Nurilla :”Ok, so you are very good for her preparation. I just want to confirm she gets
Ringer Laktat infussions, right? And I see that I have to prepare the Oxygen
therapy for sure because of that.(talk to Mrs. Setya) Mrs. Setya, can I check
your IV line please? We are controlling infections and you are ready with your
surgery, right? Doctor Stella is about to come. She is a very nive person.”
Setya :”Yes, you are right. She had supported me so much.”
Nurilla :”Ok, good. Just relax, everything is gonna be okay. We are expert to this. Just
tell me if you want me to do something for you. You can press the button here.
And I will come.”
Setya :”Thankyou so much.”
Nurilla :”Oh it has to be my duty.”
Rima :”Oh Nurse Nurill, her vital signs is as I read in her status. BP 115/80 mmHg,
Pulse 88, T 36,7 degree Celsius, RR 26 timer per minute. This is her lab result,
high in Leucosites, normal Hb and no resist or alergic to medication.”
Nurilla :”Allrights, I read it. Ok, I will monitor the vital signs again. Ad absolutely the
hydration.”
Rima :”Ok. And Doctor Stella told to me to wait for her instruction post surgery while
continue the fluid program.”
Nurilla :”Yes. Thank you Nurse Rima. Let’s back to the station and take a signature for
this.”
Rima :”Sure. Mrs. Setya, here you will accompanied by Nurse Nurilla. I will pick you
up after surgery.”
Setya :”Ok, thans a lot Nurse Rima.”
Nurse Rima and Nurse Nurilla doing hand hygiene and back to the nurse station to
complete the form of patient hand over. Then, Nurse Rima back to the Cempaka ward.
