AFFIDAVIT OF EXPLANATION

I, Tohammy P. Macadato , legal age, Filipino, married, and presently residing in Iligan City,
Philippines after having sworn in accordance with law depose and say that:

1. That with regards to the order I received from Civil Service Region 10, with regards to
the eligibility has a number D102005517 bearing my name who has been verified
spurious.

2. That the eligibility which is spurious was only given by someone to me who make us
believed that person is connected in civil service office that he presented to me his
Identification Card bearing his picture and name in the personality of LERIN.

3. I indeed believed because he presents us those credible documents. That, I am sending

my explanation letter sincerely that I am only a victim or maybe one of the victims of that
said name. I don’t have any intention to do such fictitious act that is against to the law.
And I am apologizing about what happened to this. That is the main reason why also we
confidently verified the eligibility he gives to us because of believing to him. We are
almost so long in the services of a government and we did not commit any against to the
law just now, because of somebody who abuse us and lose our confident.

4. That, it is painful to our part because that moment we decided to make on verification
believing that it will brought us good for us to recover in what happened in Marawi Siege
but it is ironically. We are asking to help us find that person.

5. This affidavit is executed to attest to the truthfulness and veracity of my statement above.
