3. Definition of safety responsibilities

ICOM Project Manager, in coordination with Client Project Manager, has the overall authority
to administer the Contractual work at respective work sites. They shall be fully supported by
the project key personnel, i.e.:

- Overall Company Representative


Managing Director - Approving Department HSE for HSE
& policies and regulations.
Operation Manager - Originator of General Directives and
Programs of the HSE Department

Site Management Responsibility


Site Management thru the Project Manager is responsible for the identification of Health,
Safety and Environmental (HSE) issues. The Project Manager shall also be responsible for
compliance of HSE requirements inherent in the project scope.

Management shall create an HSE atmosphere that provides proper communication and
trainings essential in the execution of HSE programs and procedures. The goal is to lead to
hazard recognition resulting to Zero accidents, incidents and injuries. The following key
personnel have their particular responsibilities as follows:

Project Manager/Department Head

 Know, understand and enforce all approved procedures and applicable policies on
health safety, security and environmental standards related to the scope of work of
each department/project as prescribed in the Safety Procedures Manual. Know the
requirements of relevant Saudi Arabian Government Workmen’s Regulations,
applicable clients’ Procedure on Health, Safety and Environmental protection and
other relevant local regulations required of the work, and ensure they are complied
with.
 Demonstrate the personal involvement, highest level of interest possible, to
promote a working culture and environment in which everyone shares his utmost
concern in the total prevention of injuries and losses, in support of the Company’s
Zero Incident Goal and Philosophy.
 Demonstrate active personal involvement in safety, health, security and
environmental issues through attending safety meetings, management’s periodic
safety audit’s, safety alignment sessions, weekly safety assessment walk-through,
routine daily toolbox talks, inspections, safety performance enhancing
programs/leadership training and all other activities where safety is an important
concern.
 Ensure that all accidents/incidents including first aid cases, near-miss and fires are
reported immediately to Safe & Loss Prevention Department.
 Set a personal example for employees by wearing an appropriate personal
protective equipment and clothing at all times, conducting safety inspections,
correcting on the spot-observed hazards, unsafe conditions or practices.

Site Safety Manager

 Manage overall site implementation of the client-approved project-specific Safety &


Loss Prevention Program with ICOM field work force and subcontractors. Report
administratively to the Project Manager and functionally to the ICOM Corporate
Safety and Loss Prevention Manager. His direct reporting access to the Corporate
Management through the Manager, Safety & Loss Prevention Department
assures that, in instances where safety matters and production may conflict, safety
matters receives the highest attention and priority.
 Represents ICOM in all matters pertaining to safety, security, health and
environmental issues before the client and perform all related coordination
activities.
 Be fully familiar with the project-specific emergency handling procedures and
ensure proper understanding of everyone on the project.
 Keep an organized and up-to-date record of all recordable injuries, fires, motor
vehicle accidents (MVA’s) property damage incidents, crane/heavy equipment
incidents, including near-misses, first-aid cases and all other pertinent safety
records described under section (Safety Reports & Records) of this program.
 Understand all safety, health, security and environmental related requirements of
client and client-approved Safety and Loss Prevention Program. Ensure
compliance of project work force and sub-contractors with the program.
 Conduct documented in-house pre-site deployment safety orientation or induction
course with new employees and returnees upon their arrival at project site and
before they are assigned of any task or deployed to their respective work
assignments. Inspect the work site daily to report and correct unsafe methods,
conditions or work practices.
 Conduct or facilitate safety training/refresher courses with the site work force in
conjunction with site management. Keep a record of every weekly safety meetings
on site, complete with subjects discussed and a list of attendees.
 Set a personal example

Site Engineer / Supervisor

 Organize sites so that work is carried out to the required standard with minimum
risk, to men, equipment and materials.
 Ensure utmost regard for safety and health of all personnel involved in all
operations and that equipment as well.
 Know applicable requirements of client
 Be familiar with work permit procedures.
 Give precise instruction on responsibilities for correct work methods.

 Plan and provide for good housekeeping


 Check that equipment and tools (both the power and hand tools) are maintained in
good operating condition.
 Make sure that each in-charge of his men knows how to obtain and manage
required assistance to injured, including administering first aid treatment and
summoning external assistance when called for. He should also make sure that he
has the available qualified alternate at all times to act in his absence.
 Make sure that the suitable personal protective equipment is available and that
they are properly worn.
 Release employee(s) from his group when necessary for safety training or
certification training courses
 Prohibit a person to work when physically or mentally unable to discharge his
assigned duties.
 Cooperate with the safety representatives by acting on their recommendations,
results site safety inspection or spot observations.
 Ensure giving immediate due course of action to all safety circulars / notices /
bulletins issued by Safety Department and, where required, make a report of
action taken. Prepare Job Safety Analyses (JASs) and enforce the same with his
men, if and when required such as a non-routine / high-risk activities.
 Set a personal example

Safety Engineer / Safety Officer

 Formulating, administering and reviewing as necessary the project safety plan.


 Submission of regular reports to management
 Providing advice on all safety and health matters.
 Maintaining an accident record and report system.
 Preparing reports on health and safety issues.
 Evaluating accident reports.
 Reviewing corrective actions taken to eliminate accident causes.
 Making inspection to discover and correct unsafe conditions and unsafe work
practices.
 Instructing and coordinating field operation activities, as they are always related to
overall health and safety program.
 Perform full time and continuing safety coverage/monitoring at worksites to ensure
compliance at work force.
 Report to and assist his site safety manager/supervisor in the overall
implementation of the client-approved project-specific safety and loss prevention
program and related activities.
 Coordinate for safety, health, security and environmental issues with client
counterpart in the field that concern or affect day-to-day operation.

 Be fully familiar with project-specific established emergency procedure and ensure


that the same is fully conveyed to site employees in his respective area of
coverage.
 Advise site management through his immediate superior, on the following:

 Attainable means to improve working conditions, existing work


methods and practices.
 Ways to prevent fires, injury, damage to equipment and facilities
 Contractual concerns affecting safety, health & welfare of employees
 Provision and use of special protective clothing and equipment for
specific tasks
 Predictable job hazards on site in which affected work force must be
cautioned
 Observed deviation from approved procedure / work methods or non-
conformance to the client-approved project-specific Safety, Health,
Security and Environment Program.

 Carry out site surveys/patrols to see that only safe work methods are in operation,
that health and safety requirements are being observed, and welfare facilities are
adequately and properly maintained.
 Determine the cause of any incident including near misses and recommend means
of preventing their recurrences
 Supervise / monitor daily toolbox meeting or pre-job instruction meetings of
foreman / supervisors.
 Foster within the company an understanding that injury prevention and damage
control are an integral part of business and operational efficiency.
 Monitor and ensure compliance of field work force with the work permit system.
Checked observance and enforcement of supervisors/foremen of Job Safety
Analyses on non-routine high-risk activities i.e. confined space entry, deep
excavation, critical lift, man basket operation, hydro jetting, etc.
 Set a personal example

Foreman

 Directly responsible for the safety and welfare of his workmen and the safe
execution of every task performed in his area of responsibility.
 Communicate with his subordinates potential job hazards in the immediate work
surrounding and those associated with work process, tools, materials and
equipment involved, upon assigning them and before the start of the work. Discuss
such details with emphasis on each specific craft in which his subordinates are
engaged i.e. potential fall hazards his scaffolders are likely to confront; inert

gasses and toxic fumes his welders may confront; possibility of being caught-in on
the part of his crew using rotating tools, etc.
 Train new workers under his supervision, by the way of guiding and coaching them
in existing work methods, procedures or practices, with emphasis on safety in mind
 Ensure that workers understand the job, work method desired,
equipment/tools/materials employed, and all necessary precautions to be taken
upon acceptance of and prior to performing the job. Foremen must confirm such
understanding with their respective subordinates through pre-job safety instruction
meeting.
 Be familiar with the client’s and plant procedures/guidelines applicable to the work
in which subordinate workers are engaged such as plant’s specific work permit
requirements, security and facility restrictions, equipment entry requirements, etc.
 Integrate in the day-to-day activities, site inspection/ patrol to identify and correct
any unsafe conditions or practices. Where required, document and coordinate the
safety inspection activities and findings with the site safety representatives.
 Conduct, at the start of work shift, a 5-minute toolbox meeting or pre- job safety
instruction briefing with his subordinates before the commencement of work, to
highlight job-associated hazards and safety measures required. This meeting must
be conducted at the actual location of the work to be performed in order to clearly
emphasize or relevant details associated with the job including tools / equipment /
materials to be used and the approved execution method to be employed.
 Be familiar with the applicable provisions of the client – approved project specific
safety, health & environment program, and incorporate them in the day-to-day
tasks and manning his workmen.

 Prepare initial report of accident / incident including near – accidents or near –


misses in his area of responsibility. Participate in the required accident
investigation, identify causes associated, establish permanent preventive action
and enforce them in his respective areas of supervision.
 Correct promptly observed unsafe acts such as horseplay or the taking of
unnecessary risks.
 Commend or nominate to site Safety Department for safety recognition employees
who, by action or initiative, prevent or eliminate hazards or unsafe conditions or
practices.
 Report near-misses or unsafe conditions that pose imminent danger for early
prevention of potential accidents / injuries.
 Conduct specific task safety instruction with his craftsmen assigned on hazardous
work such as operating mechanical equipment, rotating machine, pneumatic tools,
etc.

 Observe End-of-Shift Safety Check or after- work inspection before leaving his
assigned work area. This is compulsory routine for work groups engaged in hot
work i.e. welding / cutting and similar source of ignition. The possibility of
smouldering fire that caused by stray hot splatters or slogs from such activities can
take place in the last minute of work shift, hence, the need for post-job inspection.
 Be fully familiar with the project-specific emergency procedure and ensure that the
same is fully conveyed to respective workmen under his area of supervision.
 Ensure that post-accident actions or recommendation preventive actions
established after an accident occurrence are properly carried out and enforced
with his workmen.
 Set a personal example and promote positive interest towards safety before his
subordinates, by acting as a role model, through exemplifying basic compliance
i.e. proper wearing of PPE’s, avoiding shortcuts, discouraging the use of defective
tools/equipment or those of sub-standard quality, adhering to work permit
procedure and prompt correction of unsafe conditions/practices.

Male Nurse
The Nurse is a key function of an HSE organization. The immediate and
effective response of a nurse can save the life of any person who has
experienced a major injury or illness. His first aid care to people who have
experienced minor injuries and sicknesses can prevent further complications.

This prevention and medical assistance benefits both the employee and the
company. Must be certified by Saudi Arabia Ministry of Health as per
regulations to practice the health in the Kingdom. His responsibilities includes:

 Shall be responsible for administering first aid and facilitate injured or


sick personnel to obtain medical care.
 Shall monitor adverse health conditions of workers.
 Shall report to management any danger of contamination or contagious
condition.
 Shall guide workers on proper health care and management of their
sicknesses.
 Shall keep records of all injuries and sicknesses and submit a report of
these records to project management.

EQUIPMENT OPERATORS (HEOs)

 Directly responsible for the day-to-day safe operation, sound mechanical and
physical conditions of his assigned equipment.

 Ensure personal competence and awareness on the operational safety aspects of


the equipment assigned to him, including its day-to-day documented start and
periodic preventive maintenance check. Never presume if in doubt, consult
immediate superior.
 Ensure that his required license / certificate i.e. SAG license, TUV license, Iqama,
Client-issued certificate (if any) are valid and maintained in his possession while at
work.
 Be fully aware of the applicable documents that his assigned equipment must
maintain such as third party inspection certificate (Crane), plant access pass /
sticker, operating manual, capacity charts (on cranes), monthly safety inspection
sticker, monthly preventive maintenance inspection checklist, registration, current
MVPI (trucks / vehicles) as well as safety devices required such as flashlight, early
warning device (reflectorized safety cones / triangles), fire extinguisher, first-aid kit,
etc. Immediately consult responsible supervisor for any expiring, missing
requirement or lacking document.
 Ensure that the back - up alarm of his equipment is properly working at all times,
that it is audible within 30 meters distance or maintains a 90 dB sound level
minimum.
 Check at every start of work shift, oil, brake lights, water, hydraulic level gauge,
horn, back – up alarm and related operating accessories to ensure each is
properly operating. Immediately report to site equipment supervisor any deficiency
or bring the unit to the nearest repair shop / facility for appropriate corrective
action.
 Know, understand and mentally rehearse steps to be taken in the event of the
project / plant emergencies. Be constantly familiar with the project – specific
emergency handling procedure.
 Practice a quick “walk – around” each time the equipment is to be moved and
before moving the equipment, to ensure that the immediate surrounding is clear of
any personnel, hazardous conditions, materials or other equipment that can be
damaged.
 Ensure that a signed and approved hot work permit or equipment entry permit is
available before moving the equipment into an operating plant or restricted area
where such permit is compulsory. Know the capacity limit of the bridges / cross
over, heights of overhead structures or power lines before attempting to cross or
pass through with his equipment. If going to travel with load, know the actual
weight of the load and ensure it does not exceed the posted axle load capacities
on bridges (if any).
 Never refuel equipment in an operating area, close to or directly below hot work
activities such as welding / cutting. Refuel equipment in a safe open area at least
10 meters away from possible source of ignition. Ensure to shut off equipment’s
engine before and while it is being refuelled. Make sure that the proper bonding /

grounding is observed when refuelling only any machine (that grounding / bonding
connection is properly connected prior to fuelling).
 Ensure that a designated equipment spotter or watchman is provided every time
his equipment (e.g. crane, truck, pay loader including vehicles and buses),is
manoeuvring, reversing or positioning in a congested area / operating facility.
Spotter must be properly identified by means of fluorescent orange vest.
 Ensure that operator’s cab is clear of any sight obstruction such as curtains,
hanging shirts, card boards and the like which may hamper, reduce or cancel the
operator’s visibility or cause to distract the attention of the operator during the
actual operation.
 Never leave equipment operating or with its engine “switch on” unattended at any
one time.
 Never allow unauthorized personnel or riders on the equipment.
 Ensure that a required fire protection i.e. portable dry chemical fire extinguisher is
properly maintained, regularly inspected and operative at all times.
 A mobile crane operator must ensure that his crane is provided with timber or steel
outrigger protection pads of appropriate size, in his unit and used on all

unpaved non-compacted ground as well as compacted paved areas where


pavement protection is required.
 Never operate equipment in poorly illuminated areas or visibility is poor and in
harsh weather condition e.g. 22 mph wind magnitude and above.
 Always park or leave the equipment in a designated parking area. Never leave
equipment elsewhere at the construction site or operating plant.

DRIVER (HED / LED)

 Directly responsible for safe driving and operation including day-by-day


maintenance, mechanical and physical conditions of the service vehicle or unit
assigned to him.
 Ensure that a current / valid SAG (Saudi Arabian Government) driving license is in
possession at all times. Be aware of the validity / expiry date of the SAG License,
vehicle’s registration, access / pass stickers, monthly safety inspection, as well as
due date of the MVPI (Motor Vehicle Periodic Inspection) and In – house
maintenance check.
 Be familiar with all traffic rules and regulations and abide by them conscientiously.
Obey traffic lights, stop signs, speed limits and other precautions posted on
highways, roads and work site facilities. Maximum speed limit at construction site,
camp site, warehouse, lay-down yards, ESD maintenance shops and other company
facilities is 20 kph.

 Wear seat belt “every time – every trip” and enforce the same with his passenger.
Driver must insist wearing seat belts with his passengers before starting his vehicle.
 Prior to making a trip or starting his assigned service vehicle each day, he must
carry out checking for serviceability of his vehicle’s equipment and appurtenances
such as tiers, air pressure, breaks, light, fuel level, glass / mirrors, oil, water, wiper,
etc. Any observed deficiency must be reported immediately to the site equipment
responsible or referred to the nearest repair facility for immediate rectification. If a
load / material or equipment is to be transported with the vehicle, the driver must
ensure that such load is within the vehicle’s capacity limit, properly secured and
loaded in such a manner that it would not cause instability of the vehicle in transport
with respect to its weight, width and height. No portion of the load shall extend over
the sides of the vehicle. Portion that extends over the rear of the vehicle must be
marked with distinctive flag or warning light at night.
 Know, understand and mentally rehearse steps to be taken in the event of site
emergencies. Be constantly familiar with the project-specific emergency procedure.

 Observe and maintain safe distance “two second rule” concept on the road, to allow
safe breaking distance in case sudden events in the immediate approach on the
road or on traffic ahead and avoid rear collision.

 (Note that in rear collision cases, the accountability for and preventability
of the incident lies mainly on the part of the driver of the vehicle behind).

 Be familiar with the Saudi Arabian traffic violation penalties as well as with all
associated undertakings and accountabilities.
 Maintain in his possession or in his vehicle, emergency contact numbers of
appropriate sources of assistance to avail of in case of off-site road accidents such
as project site, campsite, ICOM Head Office – Human Resource Management &
Development / Saudi Government Liaison Officers.
 Note: In case of off – site road vehicular accident, concerned site
Government Relation Officer (Saudi National) must be contacted
immediately to deal with traffic authorities.
 Ensure that the back – up alarm of his equipment is properly working at all times, that
it is audible within 30 meters distance or maintains a 90 dB sound level minimum.
 Set a personal example by exemplifying professional and defensive driving attitude
on the road.

DESIGNATED COMPETENT PERSONS


 Directly responsible for the safety of the personnel, equipment and facility involved in
his assigned activity or operation in which his “competent person” designation has
specifically been defined and assigned by proper site authorities (i.e. his immediate
superior/manager /safety representatives) such as overhead crane and manlift
operators, fire watch, entry attendant, assured grounding, excavation and scaffolding
inspector, etc.
 Know and understand all safety and operational aspects involved in the specific
equipment or task assigned to him and enforce all associated safety measures and
work requirements within the confine of this given responsibility and authority. The
authority of the competent person encompasses stopping the work at any time he
feels reasonably necessary in the interest of safety.

GENERAL EMPLOYEES

 Know, understand and faithfully obey company policies, established safe work
practices, applicable procedures and job-associated work requirements.
 Has the direct responsibility for his personal safety, the safety of his colleagues
working next to him, and in maintaining his tools in proper working condition and
usage at all times.
 Wear properly, issued personal protective equipment (PPE’s) such as head/eyes/foot
protection, fall protection, face protection, respiratory protection, hand protection, etc.
and observe proper maintenance, handling and personal care of the same. Proper
use of PPEs includes prohibition from wearing non-prescribed outfits/gears that can
render the use of prescribed PPEs ineffective such as baseball cap under the hard
hat, traditional loose clothing, face/head wrappings that impair peripheral vision,
jewelleries and similar interfering or distracting outfit.
 Inspect issued tools/equipment and maintain them in clean and proper condition at all
times. Report and return defective ones.
 Initiate clean up in his assigned work area anytime the condition requires and before
the end of each work shift.
 Prevent tripping hazards, fire hazards and access obstructions in his assigned work
area, by maintaining his tools, equipment and materials in an orderly condition at all
times.
 Consult immediate superior or take a pause at work when safety is in question or
when uncertain about correct work process.
 Be familiar with basic operations of first-aid and fire-fighting equipment e.g. portable
fire extinguishers, fire hoses/hydrants available in the immediate work area.
 Do not proceed with new task unless there is clear understanding at the details about
his safe execution, methods, tools/equipment involved and the associated potential

job hazards and appropriate preventive measures have been clearly discussed by the
immediate supervisor. Seek assistance from senior authority or site safety
representatives if required. Workers have the right not to proceed with the work or
cease from on-going work or refused to proceed following a given instruction (if need
be) in the interest of safety.
 Know how to summon first aid or emergency assistance if and when needed. Consult
immediately superior or site safety representative for instructions.
 Avoid horseplay or anything that endangers oneself or others.
 Report to his immediate superior any unsafe condition observed in his work area for
immediate correction. Correct or eliminate the hazard if manageable.
 Attend compulsory weekly safety toolbox meetings, emergency safety meetings,
training (if called for) and other related activities.
 Participate in site and company safety incentive programs and awareness
enhancement schemes.
 Be constantly familiar with the project-specific emergency procedure including
locations of established safe assembly points, types of emergency alarms (if any).
Consult responsible foreman/supervisor if needed.
 Cooperate with safety representatives and site authorities by way of obeying
instruction, reminders and observing spot corrections.
 Caution any fellow employee he observes engaging in an unsafe practice or exposed
to any unsafe condition.
 Read Company’s posted information on safety, health and environmental concerns
i.e. posters, safety newsletter, bulletins, circulars, etc.
 Take pride and positive interest in maintaining safety as a personal day-to-day
concern and priority respond positively to his immediate supervisor’s behavioural
observation feedback (i.e. in the form of counselling, spot correction or
commendation), for a continuing behavioural conditioning and personal development.

HSE ADMINISTRATION
It is an essential part of every activity to have rules governing its working principles. Safety
Rules, as a vital component of ICOM operations activities, are indoctrinated to all of its
personnel before any task is given to task performers. The following safety rules are hereby
adopted:

 Safety is the responsibility of every employee.


 ICOM and its subcontractors shall comply with all the safety requirements set forth in
this HSE MANUAL. It shall be the duty of Supervisors to make known to all his task
performing all the safety rules and instructions given by safety department from time
to time.
 Non-compliance and violations of the safety provisions and instructions will render the
violator liable for disciplinary action as stipulated in this HSE MANUAL.

 The Site HSE Officer shall always be informed of any unsafe practice.
 Employees shall be given appropriate Personal Protective Equipment (PPE) for the
job they are doing.
