Carrying out a Risk Assessment – (Refer to Risk assessment procedures - FOM1701)
1. Divide the job, stage-wise, into a series of tasks.
2. Identify all the Hazards associated with each of the tasks listed.
3. List each of the effects the hazard would have, if allowed to develop to its full potential. The hazard may impact more than one entity.
4. Consider the consequences of each hazard effect, and identify the relevant band – refer to the Risk Matrix
5. Consider the probability of that occurring, and identify the relevant band – refer to the Risk Matrix
6. Note the resultant assessed risk in the risk matrix – e.g. in square B – II. Record same under the Initial Risk Column
7. Ascertain whether the risk is High, Moderate or Low as displayed by the chart shading.
8. Identify preventative and mitigating actions that would bring the risk to acceptable levels. Each hazard effect requires a corresponding action.
9. List items requiring heightened monitoring or the imposition of additional controls under column “Item/Equipment Action Required on”.
10. Review the modified risk, taking into account the preventative and mitigating actions in place, by referring to the Risk Matrix
11. Confirm that the residual risk is within acceptable levels. Record same under the Modified Risk Colum
NOTE :
1. It is recommended that an electronic copy of the risk assessment is stored for easy retrieval and subsequent editing.
2. Once prepared, a risk assessment may be archived and referred to when the job is next carried out.
3. It is recommended that the risk assessment be reviewed and updated with the crew’s latest experience once the job is completed.
4. A Toolbox meeting where this Risk Assessment, work and emergency procedures involved are discussed, must be held prior start of the job.
5. Equipment Lock out / tag out procedures to be complied with.
6. All relevant Work Permits to be filled in and duly verified prior commencement of job.
