TPI QUESTIONNAIRE

The Transgender & Personal Identity Project is studying the idea of


gender change. How did it, or does it happen, is there an awareness
of personal, internal psychological and phenomenological changes?
We are interested in the how those changes happened, the way the
trans person changed, and the ways in which the world was
experienced differently. This questionnaire will help us determine if
you are a good subject for us.

If you wish to be interviewed we need to be able to get in touch with


you once your form has been reviewed. Please understand that all
personal information will be held in the strictest of confidence in
accord with the guidelines established by the York University Office of
Research Ethics. Please fill in the following or contact purposes.

the research arising from this project will result in a deeper
understanding of several theoretical and philosophical issues involving
the transgender phenomenon.

for an interview, you can still submit a story or anecdote about your
