ALS Assessment Form 1

INDIVIDUAL LEARNING AGREEMENT


The Individual Learning Agreement (ILA) is a plan developed by the Learning
Facilitator and each Learner. The plan records what the Learner wants to learn (learning
goals), how the Learner wants to learn it (learning activities and strategies), and when the
Learner wants to learn it (timeline).

 This form is an extremely important assessment and planning record. It is an


agreement between the Learning Facilitator and the Learner through which the
Learner agrees to work towards his or her learning goals and the Learning
Facilitator agrees to help the Learner to do this.

 The information in this form can be written in the Learner’s first language (Mother
Tongue), in Filipino, or in English.

 The Learning Facilitator can complete this form if the Learner is not confident to do
so. However, the learning goals must be those selected by the Learner with the
assistance of the Learning Facilitator. The learning goals must not be those decided
upon by the Learning Facilitator.

 This form should be completed by the Learning Facilitator and Learner at enrolment
or shortly after and should be reviewed and changed, if necessary, every 2-4 weeks.

 The form must be included in the presentation portfolio.

How to Use this Form

On this form, Learning Facilitator or Learner records what, how, and when the
Learner wishes to learn. This may be written in the Learner’s first language (Mother
Tongue), in Filipino, or in English.

 Explain the purpose of the Individual Learning Agreement to the Learner.

 Assist the Learner, through small group or one-to-one discussion, to identify his or
her learning goals. In doing this, use information collected by the Document of Life
Experiences form [ALS RPL Form 1] and Summary Record of Training/Skills form
[ALS RPL Form 2].

 Provide advice to the Learner on suitable learning modules and help the Learner to
select 2-3 modules from the complete list.

 Help the Learner to fix a completion date for the selected learning modules.

 When the learning modules are completed, review the Learner’s progress. To help
you and the Learner select the next modules, use information in the Weekly
Learning Log [ALS Assessment Form 2], the Review of Learning Goals [ALS
Assessment Form 3], and Record of Module Use [ALS Assessment Form 4]. This
review should be held at least once every month.

If necessary, the Individual Learning Agreement should be modified to take account


of new learning goals, learning strategies or timelines.
