INSTRUCTIONS

1. Please tick the class that you are applying for (VIII/XI).
2. Please tick the seat that you want to apply for.
3. Age limit:
a. 12-14 years for Class VIII on 30 April of the year of admission. Six months upper waiver to Hafiz-e-Qur’an
only.
b. 15-17 years for Class XI on 30 June of the year of admission. Six months upper waiver to Hafiz-e-Qur’an only.
4. For admission in Class VIII, a candidate studying in private school, an attested copy of recognition of concerned
school by Education Department /Board of Intermediate & Secondary Education must be attached. Certificate
by the Head Master /Principal is NOT accepted.
5. Application Form and 4x photographs must be attested by the Principal/ Head of school in which the candidate is
studying. Once the Date of Birth is recorded on the Application Form, it will NOT be changed / corrected.
6. Legal Guardian of the candidate can only sign the Application Form in case the father of candidate is dead or
serving abroad.
7. Typed / photo copy of Application Form will NOT be accepted.
8. Last date for submission of application forms is:-
i. 28 October, 2017 with normal fee.
ii. 04 November, 2017 with late fee of Rs. 500/-
