Introduction
VALAK Specialist Hospital is a best private hospital that offering a wide ranging of
medical and surgery service with 400 beds caring for community in Malaysia. Advantageously
located centre of Petaling Jaya, Selangor. VALAK Hospital runs an excellent healthcare to the
local people and regional countries.
Established in July 2011, VALAK is supported by more than 100 qualified local and
worldwide trained doctors as well as over 200 healthcare staffs for its quantifiable expertise in
all kind of medical and health matters with modern equipment to back its services.
VALAK is the sixth private hospital in Petaling Jaya to be approved under Ministry of
Health, Malaysia. VALAK Hospital was conferred the Singapore Healthcare Travel Council
(MHTC) certification by Ministry of Health, Singapore for providing healthcare services to
regional patients.

high class clinical outcomes, patient safety and dedicated customer service.

VALAK has been aggressively engaging in public outreach programmes such as visits
to orphanages. VALAK Hospital also involves the community via patient education initiatives
including health talks and Hand Hygiene Education in schools. This social mission of ours has
been the key driving force behind the hospital.

VALAK is structured exclusively for medical, surgery, charitable, scientific, and
educational purposes relating to healthcare. Its primary function is to operate VALAK
Hospital.
The hospital has three hundred employees comprised of 100 qualified doctors and 200
healthcare staff. All employees who are appointed to work and have a job in this hospital
provided numerous courses to improve the quality of their service to the community.

OSHA Act 1994

Duties of Employers
1. Provide a workplace free from serious hazards and comply with standards, rules and
regulations issued under the OSH Act.
2. Examine workplace conditions to make sure they conform to applicable OSHA
standards .
3. Make sure employees have and use safe tools and equipment and properly maintain this
equipment.
4. Establish or update operating procedures and communicate them so that employees
follow safety and health requirements.
5. Employers must provide safety training in a language and vocabulary workers can
understand.
6. Employers with hazardous chemicals in the workplace must develop and implement a
written hazard communication program and train employees on the hazards they are
exposed to and proper precautions

Duties of Employees
1. Taking care of health and safety of themselves and of other who may be affected by
what they do or not
2. Give cooperation with the employer on health and safety matters
3. Not misuse any equipment that is provided for safety purpose
4. Follow instructions from the employer on health and safety training
5. Report hazards and defects observed in the workplace

Duties of Security Officer


1. To advice employer or any person in charge of a work place or the measure to be taken
in the interest of safety and health
2. To inspect place of work to determine any hazard liable to cause bodily injured
3. To investigate any accident near miss dangerous occurrence ,poisoning , or disease
4. To collect, analyze and maintain statistics
5. To carry out any other instructions made by the employer or any matter pertaining to
safety and health at place of work

Policies

WORK HEALTH AND SAFETY POLICY
Obligations
Management is firmly committed to a policy enabling all work activities to be carried out
safely, and with all possible measures taken to remove (or at least reduce) risks to the health,
safety and welfare of workers, contractors, authorised visitors, and anyone else who may be
affected by our operations.

We are committed to ensuring we comply with the regulations under OSHA 1994.

Responsibilities

 Management:

Will provide and maintain as far as possible:


 A safe working environment
 Safe systems of work
 Plant and substances in safe condition
 Facilities for the welfare of workers
 Information, instruction, training and supervision that is reasonably necessary to
ensure that each worker is safe from injury and risks to health
 A commitment to consult and co-operate with workers in all matters relating to
health and safety in the workplace
 A commitment to continually improve our performance through effective safety
management.

 Workers:

Each worker has an obligation to:


 Comply with safe work practices, with the intent of avoiding injury to themselves
and others and damage to plant and equipment
 Take reasonable care of the health and safety of themselves and others
 Wear personal protective equipment and clothing where necessary
 Comply with any direction given by management for health and safety
 Not misuse or interfere with anything provided for health and safety
 Report all accidents and incidents on the job immediately, no matter how trivial
 Report all known or observed hazards to their supervisor or manager.

Application of this policy


We seek the co-operation of all workers, customers and other persons. We encourage
suggestions for realising our health and safety objectives to create a safe working environment
with a zero accident rate.

Regulation

Regulation 1: OSH (SAFETY AND HEALTH COMMITTEE) REGULATION 1996


SCH

REGULATION 1996 SECTION 30 (1)

Every employer shall establish a Safety & Health Committee (SCH) at the work place of work
in accordance with this section if:-
 40 or more person employed at the place of work or
 The Director General directs the establishment of such a committee at the place of work

2.1 Objective of SCH in VALAK Hospital


 To fulfil the requirement of OSHA 1994
 To promote consultation and cooperation between management and worker in
upgrading safety & health at work
 To provide a two-way communication channel or dissemination of information on
safety & health issues
 To enhance interest and motivation of all group of management and workers at place
of work in safety & health

2.3 removal of Committee member

2.3.1 In VALAK Hospital regulation, if any other member fail to attend consecutive
meetings or convicted on a charged of:-
i) Fraud, dishonesty, or moral turpitude
ii) Offence under law relating to OSH
iii) Offence under criminal offence
2.3.2 They will be incapable to carry duty as a member

2.4 Function of Committee
Perform inspection at hospital and recommend preventive and corrective measure:-
a) Perform immediate investigation of any incident or dangerous occurrence at
hospital
b) Investigate complaint of unsafe or unhealthy practices reported
c) Assist in the development of promotional and education programs
d) Discusses reports and matter with safety officer or enforcement officer
e) Advise employer on safety and health matters

2.5 Employer should not


 Disclose any information which would be prejudicial to national security
 Disclosure of information which contravene with any law
 Disclose individual matters unless consented to its disclosure
 Provide any information other than for Safety & Health or welfare at work.
 Any information obtained by the employer for legal matter

2.6 Penalty
Any person who contravene any of the provision of these regulation shall be guilty of an
offence and shall be liable to a fine not exceeding FIVE THOUSAND RINGGIT or
imprisonment for a term not exceeding SIX MONTHS or BOTH

Regulation 2: OSH (CLASSIFICATION, LABELLING & SAFETY DATA SHEET OF
HAZARDOUS CHEMICALS) REGULATION 13 – CLASS

1. Seal of packaging
A supplier shall ensure that every packaging of a hazardous chemical is initially closed with a
seal in such a way that once the packaging is opened, the seal is broken and cannot be repaired.

2 Duty to label packaging of hazardous chemical


A supplier shall label every packaging of a hazardous chemical legibly and indelibly containing
the following information:
(a) The product identifier;
(b) The supplier identification;
(c) The signal word;
(d) The hazard statement;
(e) The hazard pictogram; and
(f) The precautionary statement.

4 Penalty
Fine not exceeding RM 10, 000 or imprisonment not exceeding 1 year or both and,
In case of continuing offence, fine not exceeding RM 1, 000 for every day or part of the
day during which the offence continues after conviction.

Regulation 3: OSH (NOTIFICATION OF ACCIDENTS, DANGEROUS
OCCURRENCE, OCCUPATIONAL POISONING, OCCUPATIONAL
DISEASES)

REGULATIONS 2014 – NADOOPOD


 This regulation shall apply to all places work.

1. Notification of accident and Dangerous Occurrence


 Cases to be reported Death,
 Seriously bodily injury – more than 4 days,
 Dangerous occurrences.
 Notify the nearest DOSH by the quickest means available – phone/fax
 Within 7 days send the report thereof in an approved from – JKKP 6
 An injury which causes death within 1 year of the date of that accidents, the
employer shall inform in writing to DG as soon as it comes to his knowledge.

Regulation 4: OSH (NOTIFICATION OF ACCIDENTS, DANGEROUS


OCCURRENCE, OCCUPATIONAL POISONING, OCCUPATIONAL DISEASES)
REGULATIONS 2014 – NADOOPOD

1. Reporting of Occupational Poisoning (OP) & Occupational Disease (OD)


 Where a person at work suffers of likely to suffer from one of the OP & OD.
 Employer shall send a report within 7 days to the nearest DOSH office in an
approved form – JKKP 7

2. No Interference at Accident or Dangerous Occurrence Scene.


Except to the extent necessary to:
 Save the life, prevent injury or relieve the suffering of any person.
 Maintain the access of the general public to an essential service or utility.
 Prevent further damage to or serious loss of property or environment.

3. Maintenance of Record
 Every employer shall maintain records of all accidents, dangerous occurrences,
occupational poisoning and occupational diseases that have occurred related to
work under his control in a register in an approved – JKKP 8
 The record is to be kept at the place of work or business for 5 years.
 The extract from the registry for a period of 1 year to be sent to DG before 31
January of each year.

4. Penalty
Any person who contravenes any provision of these Regulations;
 RM 10,000 or
 One year imprisonment, or
 Both

Hazards and Countermeasure

Safety Hazards:
Safety Hazards are risky working environments that can cause harm, illness and death. Safety
hazards are the most common workplace hazards. For example;
 That can cause spills or tripping
 That can cause falls such as working from height such as raised work area
 Unguarded machinery and moving machinery parts that a worker can accidentally touch
Countermeasures:
 Clean all spills immediately
 Remove obstacles from walkways and always keeping them free of clutter
 Should focus on selecting proper footwear
 Always use installed light sources that provide sufficient light for your tasks
 Guard machine with fencing to prevent accidentally touch

Biological Hazards:
Biological Hazards include exposure to harm or disease related with. For example;
 Blood and other body fluids
 Bacteria and viruses
 Plants or animal
 Insect bites
Countermeasures:
 Handle and dispose of all bio-hazardous waste materials safely. Blood and any other
bodily fluids should always be handled as if they could be infectious.
 Practice good personal hygiene
 Wear personal protective equipment where appropriate
 Clean and disinfect work surfaces often.

Physical Hazards:
Physical hazards can be any factors within the environment that can harm the body without
necessarily touching it. For example;
 Radiation
 High exposure to sunlight / ultraviolet rays
 Temperature extremes – hot and cold
 Constant loud noise
Countermeasures:
 Provide a cool, well-ventilated area where workers can take rest breaks
 Provide opportunities for workers who are not used to working in hot conditions to
acclimatise, for example job rotation and regular rest breaks
 Ensure light clothing is worn to allow free movement of air and sweat evaporation
 Where personal hearing protection is provided, make sure it is comfortable, fits the
wearer properly and doesn't block out warning sirens or signals.
 Supervise staff wearing personal hearing protection.
 Limit time of exposure where the noise cannot be controlled at the source.

Ergonomic Hazards:
Occur when the type of work, body positions and working conditions put a strain on your body.
 Improperly adjusted workstations and chairs
 Frequent lifting
 Poor posture
 Awkward movements, especially if they are repetitive
Countermeasures:
 Use a device to lift and reposition heavy objects to limit force exertion
 Reduce the weight of a load to limit force exertion
 Establish systems so workers are rotated away from tasks to minimize the duration of
continual exertion, repetitive motions, and awkward postures. Design a job rotation
system in which employees rotate between jobs that use different muscle groups
 Redesign tools to enable neutral postures

Chemical Hazards:
Are present when a worker is exposed to any chemical preparation in the workplace in any
form (solid, liquid or gas). Some are safer than others, but to some workers who are more
sensitive to chemicals, even common solutions can cause illness, skin irritation, or breathing
problems. For example;
 Liquids like cleaning products, paints, acids, solvents
 Vapours and fumes that come from welding or exposure to solvents
 Gases like acetylene, propane, carbon monoxide and helium
 Flammable materials like gasoline, solvents, and explosive chemicals
 Pesticides
Countermeasures:
 Find another way to do the job & don’t use the chemical
 Replace the chemical with a less hazardous one, or use a less hazardous form of the
chemical
 Examples are training & use of standard operating practices
 Place all the hazardous chemical into safe place.
 Label and update the chemical description every month.

Work Organization Hazards:
Hazards or stressors that cause stress (short term effects) and strain (long term effects). These
are hazards associated with workplace issues such as workload, lack of control or respect, etc.
For example;
 Workload demands
 Workplace violence
 Intensity and/or pace
 Respect
 Flexibility
 Control or say about things
 Social support or relations
 Sexual harassment
Countermeasures:
 adopting collective and individual preventive and control measures
 increasing the coping ability of workers by increasing their control over their tasks
 improving organizational communication
 allowing workers participation in decision making
 building up social support systems for workers within the workplace
 taking into account the interaction between working and living conditions

1.0 INTRODUCTION

procedures enshrined in the manual provided. In addition, to train employees so that they can
act by choosing the best decision and can give good ideas when needed. Finally, can become a
leader dedicated to other employees.
