Introduction
VALAK Specialist Hospital is a best private hospital that offering a wide ranging of
medical and surgery service with 400 beds caring for community in Malaysia. Advantageously
located centre of Petaling Jaya, Selangor. VALAK Hospital runs an excellent healthcare to the
local people and regional countries.
Established in July 2011, VALAK is supported by more than 100 qualified local and
worldwide trained doctors as well as over 200 healthcare staffs for its quantifiable expertise in
all kind of medical and health matters with modern equipment to back its services.
VALAK is the sixth private hospital in Petaling Jaya to be approved under Ministry of
Health, Malaysia. VALAK Hospital was conferred the Singapore Healthcare Travel Council
(MHTC) certification by Ministry of Health, Singapore for providing healthcare services to
regional patients.

OSHA Act 1994
Duties of Employers
1. Provide a workplace free from serious hazards and comply with standards, rules and
regulations issued under the OSH Act.
2. Examine workplace conditions to make sure they conform to applicable OSHA
standards .
3. Make sure employees have and use safe tools and equipment and properly maintain this
equipment.
4. Establish or update operating procedures and communicate them so that employees
follow safety and health requirements.
5. Employers must provide safety training in a language and vocabulary workers can
understand.
6. Employers with hazardous chemicals in the workplace must develop and implement a
written hazard communication program and train employees on the hazards they are
exposed to and proper precautions
Duties of Employees
1. Taking care of health and safety of themselves and of other who may be affected by
what they do or not
2. Give cooperation with the employer on health and safety matters
3. Not misuse any equipment that is provided for safety purpose
4. Follow instructions from the employer on health and safety training
5. Report hazards and defects observed in the workplace

Obligations
Management is firmly committed to a policy enabling all work activities to be carried out
safely, and with all possible measures taken to remove (or at least reduce) risks to the health,
safety and welfare of workers, contractors, authorised visitors, and anyone else who may be
affected by our operations.
We are committed to ensuring we comply with the regulations under OSHA 1994.

Regulation
Every employer shall establish a Safety & Health Committee (SCH) at the work place of work
in accordance with this section if:-
40 or more person employed at the place of work or

2.3.1 In VALAK Hospital regulation, if any other member fail to attend consecutive
meetings or convicted on a charged of:-
i) Fraud, dishonesty, or moral turpitude
ii) Offence under law relating to OSH
iii) Offence under criminal offence
2.3.2 They will be incapable to carry duty as a member

2.4 Function of Committee
Perform inspection at hospital and recommend preventive and corrective measure:-
a) Perform immediate investigation of any incident or dangerous occurrence at
hospital
b) Investigate complaint of unsafe or unhealthy practices reported
c) Assist in the development of promotional and education programs
d) Discusses reports and matter with safety officer or enforcement officer
e) Advise employer on safety and health matters
2.6 Penalty
Any person who contravene any of the provision of these regulation shall be guilty of an
offence and shall be liable to a fine not exceeding FIVE THOUSAND RINGGIT or
imprisonment for a term not exceeding SIX MONTHS or BOTH

Regulation 2: OSH (CLASSIFICATION, LABELLING & SAFETY DATA SHEET OF
HAZARDOUS CHEMICALS) REGULATION 13 – CLASS
1. Seal of packaging
A supplier shall ensure that every packaging of a hazardous chemical is initially closed with a
seal in such a way that once the packaging is opened, the seal is broken and cannot be repaired.

3. Maintenance of Record
Every employer shall maintain records of all accidents, dangerous occurrences,
occupational poisoning and occupational diseases that have occurred related to
work under his control in a register in an approved – JKKP 8
The record is to be kept at the place of work or business for 5 years.
The extract from the registry for a period of 1 year to be sent to DG before 31
January of each year.
4. Penalty
Any person who contravenes any provision of these Regulations;

Hazards and Countermeasure
Safety Hazards:
Safety Hazards are risky working environments that can cause harm, illness and death. Safety
hazards are the most common workplace hazards. For example;
That can cause spills or tripping
That can cause falls such as working from height such as raised work area
Unguarded machinery and moving machinery parts that a worker can accidentally touch
Countermeasures:
Clean all spills immediately
Remove obstacles from walkways and always keeping them free of clutter
Should focus on selecting proper footwear
Always use installed light sources that provide sufficient light for your tasks
Guard machine with fencing to prevent accidentally touch

Biological Hazards:
Biological Hazards include exposure to harm or disease related with. For example;
Blood and other body fluids
Bacteria and viruses
Plants or animal
Insect bites
Countermeasures:
Handle and dispose of all bio-hazardous waste materials safely. Blood and any other
bodily fluids should always be handled as if they could be infectious.
Practice good personal hygiene
Wear personal protective equipment where appropriate
Clean and disinfect work surfaces often.

Physical Hazards:
Physical hazards can be any factors within the environment that can harm the body without
necessarily touching it. For example;
Radiation
High exposure to sunlight / ultraviolet rays
Temperature extremes – hot and cold
Constant loud noise
Countermeasures:
Provide a cool, well-ventilated area where workers can take rest breaks
Provide opportunities for workers who are not used to working in hot conditions to
acclimatise, for example job rotation and regular rest breaks
Ensure light clothing is worn to allow free movement of air and sweat evaporation
Where personal hearing protection is provided, make sure it is comfortable, fits the
wearer properly and doesn't block out warning sirens or signals.
Supervise staff wearing personal hearing protection.
Limit time of exposure where the noise cannot be controlled at the source.

Ergonomic Hazards:
Occur when the type of work, body positions and working conditions put a strain on your body.
Improperly adjusted workstations and chairs
Frequent lifting
Poor posture
Awkward movements, especially if they are repetitive
Countermeasures:
Use a device to lift and reposition heavy objects to limit force exertion
Reduce the weight of a load to limit force exertion
Establish systems so workers are rotated away from tasks to minimize the duration of
continual exertion, repetitive motions, and awkward postures. Design a job rotation
system in which employees rotate between jobs that use different muscle groups
Redesign tools to enable neutral postures

Chemical Hazards:
Are present when a worker is exposed to any chemical preparation in the workplace in any
form (solid, liquid or gas). Some are safer than others, but to some workers who are more
sensitive to chemicals, even common solutions can cause illness, skin irritation, or breathing
problems. For example;
Liquids like cleaning products, paints, acids, solvents
Vapours and fumes that come from welding or exposure to solvents
Gases like acetylene, propane, carbon monoxide and helium
Flammable materials like gasoline, solvents, and explosive chemicals
Pesticides
Countermeasures:
Find another way to do the job & don’t use the chemical
Replace the chemical with a less hazardous one, or use a less hazardous form of the
chemical
Examples are training & use of standard operating practices
Place all the hazardous chemical into safe place.
Label and update the chemical description every month.

Work Organization Hazards:
Hazards or stressors that cause stress (short term effects) and strain (long term effects). These
are hazards associated with workplace issues such as workload, lack of control or respect, etc.
For example;
Workload demands
Workplace violence
Intensity and/or pace
Respect
Flexibility
Control or say about things
Social support or relations
Sexual harassment
Countermeasures:
adopting collective and individual preventive and control measures
increasing the coping ability of workers by increasing their control over their tasks
improving organizational communication
allowing workers participation in decision making
building up social support systems for workers within the workplace
taking into account the interaction between working and living conditions

1.0 INTRODUCTION

The program will be implemented for employees is to build effective working on their
careers. After that, the program is to train them so they can do a good job following the
procedures enshrined in the manual provided. In addition, to train employees so that they can
act by choosing the best decision and can give good ideas when needed. Finally, can become a
leader dedicated to other employees.
