TEMPLATE 1: 2017 ACCOMPLISHMENT REPORT

1. Based on the enhanced Key Result Areas (KRA) of the DRRMS, fill up the 2017 Accomplishment. In cases where accomplished Program/Projects/Activities (PPAs) of
the region/division did not match any of the KRA, include it in the last row, Others.
2. Please refer to the footnotes for the definition of terms and further instructions. Kindly refer to the list of activities and outputs of the 2017 Plans created during the
2016 Evaluation, Planning and Consultation (EPC) Workshop and the monitoring templates of the DRRMS-downloaded funds (please access the monitoring templates
here: bit.ly/2017DRRMSTEMPLATES).
3. All divisions are to send the completed accomplishment report to the respective regions for the regions consolidation, including drrmo@deped.gov.ph for their
information. Regions should consolidate all the reports (including their own) and use the same template below in the consolidation. The 2017 Regional
Accomplishment Report should then include all accomplishment from the regions and divisions.
4. The 2017 Regional Accomplishment Report should be the basis of the regions in their presentation during the 2017 DRRMS Year-End Evaluation and Planning. The
Regional DRRM Coordinator and their respective Division DRRM Coordinators may convene before the schedule of the 2017 DRRMS Year-End Evaluation and Planning
to consolidate and harmonize their 2017 Accomplishments.

Best Practices/Innovation Best Practices are the ones which meet 4 out of 6 criteria: (1) At least 75% of schools are implementing; (2) At least 3 years of implementation; (3) Structures/mechanism in place to
sustain implementation; (4) Monitoring and evaluation tools exist to assess effectiveness of implementation; (5) Policies localized to ensure adoption/adaption; and (6) Outcome/ results have been demonstrated.
On the other hand, Innovation has the potential to be a Best Practice.

Output Direct effects of a certain activity: These may include no. of participants (total no. of participants and their designation), activity output (materials; policies, plans, etc. produced by the participants),

Outcome Results An aftermath effects of a certain activity that may result into long-term impact. Such results may include cascading of the activity down the line, organized group of people for a certain cause,
implementation of a certain plan produced during the conduct of the activity, etc.
