Dear Sir/Madam,

REQUEST FOR QUOTATION FOR SUPPLY AND DELIVERY OF BioMini (BIOMETRIC

DEVICE)

The Corporation wishes to procure the service of Supply and delivery of BioMini
(Biometric Device) for which this request for quotation is issued.

1. Bids can be sent via the methods below;-

Via email address to (rchumba@cdsckenya.com and jkitaka@cdsckenya.com )

with the subject line clearly indicating QUOTATION FOR SUPPLY AND
DELIVERY OF BioMini (BIOMETRIC DEVICE) - CDSC/RFQ 004/2017 Bids
should be sent on or before 30th October 2017.

Via physical delivery at our offices on 10th floor, Nation Center Building, Kimathi
Street. Bids should be dropped at our offices on or before 30th October 2017.
The envelope containing the quotation must be clearly marked at the top

2. Your quotation must be valid for a period of thirty (30) days from the closing date of
the request for quotation.

3. The quotation shall be completed and signed by an authorised representative of the

Supplier.

4. Prices quoted MUST be inclusive of VAT and all other costs applicable.

5. In the case of any arithmetical discrepancy between the Unit Rate and the Total
Amount quoted, then the Unit Rate shall prevail both for the evaluation of quotations
and for the subsequent Purchase Order.

6. Failure to observe the above conditions shall lead to automatic disqualification of the
bidder.

7. There will be no public opening of quotations; the Purchaser is not bound to accept the
lowest quotation and reserves the right to accept or reject any or all the quotations
without assigning any reason whatsoever.

disqualified.
