TETANO

Definicin: El ttano es una enfermedad aguda provocada por

las neurotoxinas producidas por la bacteria Clostridium
tetani un bacilo anaerbico Gram Positivo, que se encuentra comnmente en el
suelo y las heces, cuyos efectos en el sistema nervioso generan espasmos e
inestabilidad del sistema autnomo. No es contagioso.

Sntomas:

-Espasmos musculares.

-Rigidez en el cuello.

-Opisttonos.

-Fiebre.

Tratamiento:

La administracin de suero o vacuna antitetnica, acompaado de penicilina. En

caso de alergia a la penicilina se utilizar metronidazol. Requiere observacin
por semanas hasta controlar la enfermedad.

Caso clnico:

Ahora presentaremos el caso de la seora Ana de trente y cinco aos infectada

con la bacteria Clostridium tetani.

(La paciente llega al centro mdico y se encuentra con la enfermera)

P: Disculpe podra ayudarme, me siento mal.

E: Tome asiento, ahora cunteme, qu le ocurri?.

P: Hace tres das me hice una herida en el brazo con un alambre, la herida no
sana, me duele todo el brazo y el cuello.

E: Antes de referirle con el medico djeme tomarle la temperatura y limpiarle la

herida.

P: Est bien, pero tenga cuidado por favor, ya intente curar la herida y duele
mucho.

(La enfermera le toma la temperatura )

E: Tu temperatura esta elevada. Limpiar la herida inmediatamente.

(La enfermera limpia la herida y el paciente muestra signos de dolor)

E: Me puede dar sus datos personales por favor, Cul es su nombre?.

P: Mi nombre es Ana Lpez Ortega

E: cul es tu edad?

P: Tengo treinta y cinco aos.

E: cul es tu peso?

P: Sesenta kilos

E: Cul es tu altura?

P: ciento sesenta centmetros.

E: Puedo tomar la presin?

P: Est bien.

(Le toma la presin y anota)

(La enfermera se dirige hacia el consultorio del doctor, y toca la puerta)

E: Doctor le traigo el historial de la seora Ana. (Pone el historial en el escritorio)

D: Gracias (el doctor revisa el historial). Seora Ana pase por favor.

P: Buenos das doctor

D: Buenos das seora, tome asiento por favor.

P: Gracias.

D: Cunteme ,qu paso?.

P: Doctor, hace cuatro das me hice una herida en el brazo con un alambre, me
he sentido mal porque tengo calambres, la espalda me duele, no puedo mover
bien el cuello.

D: Veo que tiene la temperatura muy alta, Hace cuantos das esta as?

P: La fiebre empez hace unos dos das, pero los calambres son ms recientes.
Sobre todo el dolor de espalda.

D: Djeme revisarle por favor (el doctor la revisa, revisa la herida). Levante el
brazo por favor.

P: Como puede ver doctor no puedo, me duele.

D: Ahora intente mover la cabeza hacia los lados.

P: Solo puedo moverlos hasta cierto punto.

D: Ana usted tiene los signos y sntomas del ttano, lo ms probable es que el
alambre con el cual se hizo la herida, haya estado infectado con la bacteria
causante de esta enfermedad.

P: Y es muy grave, o se puede tratar con algn medicamento?.

D: Tranquila Ana, usted llego a tiempo al hospital, podemos tratarla gracias a

que la enfermedad est iniciando, en primer lugar le recetare diazepam para
evitan las contracciones musculares y la enfermera le pondr una dos vacunas
en ambos hombros.

P: Odio las vacunas, pero por mi salud tendr que ser fuerte.

D: La enfermedad necesita cuidados especficos por lo cual debe ser

hospitalizada, para poder controlar la enfermedad.

(El doctor entrega la receta a la paciente)

P: Gracias (la paciente sale del consultorio). Seorita el doctor me dio esta
receta.

E: Le aplicare dos vacunas en los hombros uno en la derecha y otra en la

izquierda. Permtame por favor (le aplica las vacunas)

P: Seorita por cuantos das tomar el medicamento?

E: Por ahora los medicamentos sern brindados por cuatros das ya que estar
en observacin.

Prevencin: Debemos estar vacunados contra el ttano, evitar estar en contacto

con elementos cortantes como clavos y alambres, en caso de un corte limpiar
inmediatamente la herida y acudir a un centro mdico.

TETANUS
