CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

The Oroquieta Adventist Education Center Inc., represented by ROBERT P.

LAMPARAS, hereinafter referred to as the SECOND PARTY.

DAISY JANE CAHANAP, Filipino, married of legal age and residing at Lower Lamac,
Oroquieta City, the FIRST PARTY.

WITNESSETH

WHEREAS, the SECOND PARTY is in need of a person who can ably render/perform
the services as Teacher in this school.

WHEREAS, the FIRST PARTY is ready and willing to render/perform the aforesaid
services.

NOW THEREFORE, for in consideration of the foregoing premises and their mutual
covenant hereinafter set forth, the parties hereto agrees, as follows:
1. The FIRST PARTY shall render/perform contractual services assigned to her/him by the
SECOND PARTY, for eight (8) hours during school days and report during Saturdays and
Sundays wherein the exigency of the services is needed, with the following duties and
responsibilities, to wit:
a. Teaches the GRADE IV pupils using the K-12 curriculum as prescribed by the
Department of Education.
b. Enrolls pupils.
c. Evaluates pupils progress and provides various experiences for their continuous
development.
d. Works with other school personnel, parents and community for the total
development of the children.
e. Keeps pupils and school records and prepare required reports.
f. Supervises curricular and co-curricular projects and activities of the children.
g. Coordinates and cooperates with other school activities or projects.
h. Provides guidance services to the pupils
i. Does related works.
2. The SECOND PARTY, in consideration of the said services, shall pay the FIRST PARTY
the amount of 7,000 per month for work actually rendered as basic salary which shall be
made every end of the month.
3. It is understood that the FIRST PARTY is engaged in contractual basis by which reason
he/she sall be only entitled to benefits of SSS, Pag-ibig and Philhealth.
4. This contract shall be effective for the period from June, 2017 to May, 2020, subject to the
extension as may mutually agree upon by the parties.
5. This contract may be terminated by the second party at any time with or without cause even
prior to its expiry date upon written notice to that effect at least five (5) days in advance.
