Journal #2- Week 4

Based on my reflection after my first journal concerning my first direct care week, I have

come to the conclusion that I have improved in direct care with my patients, as well as other

skills. I have performed tasks such as wound care, patient care, observed administration of

medications and performed head-to-toe assessments.

My patients these last few weeks have all been diverse in ways that have allowed me to

grow as a nursing student. It allowed me to research their admitting diagnosis, their medical and

surgical histories, and their medications. As I started getting used to preparing for my pre-

planning forms and for my days at clinical, I realized that I have become very familiar with my

clinical site and the patients. These past few weeks I have performed regular tasks such as

assisting my patients and other non-regular patients with assistance to their bathrooms, peri-care,

clothing management and dressing. In addition, I had the opportunity to practice my class skills

of wound care with three different patients, while being overlooked by a CNA, to ensure that it

was performed correctly for the patients assurance.

One of my biggest responsible tasks happened this week. I was assigned to a nurse in

order to learn the process of medication management and administration. Although, I was not

allowed to administer the medications to the patients myself, I was able to learn how serious it is

that medication administration and management is to be done carefully and responsibly. The

administration of Narcotics however, take more steps in order to administer to the patients. The

medications are usually locked but if the patient needs a narcotic the Nurse usually has to take

out another key that is specifically for a narcotics drawer and record on a binder how much it

was taken out and how much it was left in the drawer in order to account for all narcotics when it

is given to the next Nurses shift.


In conclusion, I have enjoyed every clinical day that I have had because it has taught me

many skills that I will need when I become a nurse. It has also taught me the patience and

amount of responsibility that is needed in order to keep my safety and most importantly my

patients safety. Being a nurse involves being attentive with daily responsibilities and the

patients but also by making sure your co-workers are keeping up with their responsibilities as

well. Making sure their work coincides with the policies will ensure that all patients are safe as

well as to maintain a safe and responsible facility. Working alongside VMRCs CNAs and other

nurses have showed me how important that step can be.
