Greetings!

This representation would like to extend my thanks to your good office as our partner
government unit for the successful operation of 33 beneficiaries of the cleft palate
operation held at Kidapawan City Hospital on January 9, 2017.

In this light, We are fortunate again that Smile Train- Advance Craniofacial Project
Philippines Inc. ( ACPPI) had decided for its 2nd wave to perform another Free Cleft
Lip and Surgical Mission in the 2nd District of the Province of Cotabato, in partnership
with this office and your office.

Anent to this, May I ask again for assistance to your office to shoulder the hotel
accomodation and meals of the 13 staffs of ACPPI who will be doing the surgery during
the 4 day activity, starting June 18, Sunday to June 23 Friday. Further requesting 1
room in the City Hospital for the place of the surgical operation.

My office will shoulder other necessary expenses before, during and after this activity.

I am grateful of your continued support and partnership for the publick service. Thank
