I, NABEL LOMABAO ABDULLAH, Filipino, married to NAIMA MANDI ABDULLAH, both of legal age and resident of
Ideal Subdivision, Quezon City after having been duly sworn in accordance with law hereby depose and say that:

1. I bought a parcel of land on January 06, 2014. The said land has a building and in the Deed of Sale its
address is 324 Elizondo St., Quiapo, Manila.
2. On the other hand, in the record of Meralco, the building with service ID# 338954380101 under
the name of Siu Y Woo has an address of 341 Elizondo St., Quiapo, Manila City
3. Both addresses pertain to one building in the same location.
4. I am executing this affidavit to attest to the truth and for whatever legal purposes it may
serve.
