Govt. Instructions. The candidate is advised to get the signature of the Adda Incharge on this slip before boarding the bus to avoid

Note: - The facility of free traveling in HR. Roadways buses is valid within Haryana territory and is admissible to the
candidates who are resident of Haryana State.
IMPORTANT INSTRUCTION FOR THE CANDIDATES (Please read instructions carefully)

Candidates should bring with him/her the following original documents (With attested photocopies)

1. Interview will be held of those candidates who have qualied for interview after Scrutiny of Documents as per
result notice dated 10.10.2017.
2. Interview will be conducted of only eligible candidates after verication/scrutiny of documents.
3. Candidates must bring;-
a. Downloaded application form and original certicates/diploma/degree from matriculation onwards,
experience certicate with salary and date of issue (if so required) PHC/Caste/Haryana Resident
Certicate, DESM Eligibility certicate, ESM discharge book etc. forverication along with photocopy of
each on or before cuto date of the said advertisement.
b. Proof of date of birth.
c. Original Commissions copy of proof of fee.
4. No second chance will be given to the absent candidates. No further correspondence will be entertained in this
regard.
5. Mere issuance of Admit card does not entitle a candidate to take his/her interview. Candidature of the applicant
is provisional subject to his/her fulllment of eligibility conditions on cut-o date as per condition of
advertisement.
6. If at any stage it is detected that a candidate does not fulll the requisite qualications prescribed for the post
till the date of submission of application form, or is ineligible on any other ground, his/her candidature will be
cancelled.
7. DESM candidate will bring Self Declaration to the eect that he has not seized to be a DESM till today or any
other member of his/her family has not taken benet of DESM in Govt. Service.
8. Use of mobile, pager and other electronic/communication devices is strictly prohibited during the process of
verication/scrutiny of documents-cum-interview. If any candidate is found in possession of mobile phone or
any other electronic gazette, his/her candidature will be cancelled.
9. The attendance in the interview will be marked using Bio-Metric Attendance process by capturing the thumb
impression.
