SENATOR HEATHER SOMERS SENATE REEUBLICAN MAJORITY Perce, State of Conmecticut «lr eimeicnct SENATE een, tober 10,2017 ‘The Honorable Miriam Delphin-Rittmon, Department of Mental Health & Addiction Services 4410 Capitol Avenue Hartford, CT 06134 Dear Commissioner Delphi-Rittmon: ‘As the Co-Chairman of Public Health Committee and in light ofthe recent arrests of nine individuals and suspensionof 31 individuals who are curently employed by the State of Connecticut atthe Whiting Forensic Facility, 1 would respectfully request the following list of documents and oF information, jon of Healtheare 1) Copies ofthe last audit completed by Joint Commission on Accret Organizations 2) Copies of any internal or third party audits performed on the Whiting facility oat the request of your agency for 2011 to 2016. 3) Copies of any interal or external surveys done by CVE forthe last 10 years 4) The Whiting Forensic organizational chart forthe last 10 years along withthe job descriptions for eae: employee, 2006 to Present. 3) Standard Operating Procedures forall management and employees eoncerning the day to-day operation of Whiting Forensic, 2006 to Present 6) Policy and on emplayce hiring, scheduling, diseipline, transfer and termination for Whiting Forensic. 7) Policies and any intemal or external reports governing voluntary and mandatory overtime for 2009 102016, 8) Accounting of all cumulative overtime payments - voluntary and mandatory ~ foreach ‘employee at Whiting Forensic ftom 2011-2016. 9) Alline-by-line accounting by facility on the CVH campus including salaries, benefits, ‘overtime (broken out by mandatory and voluntary), 2011-2016. 10) All evenue totals generated from Medicaid reimbursement by facility at CVH from 2011-2016. 11) Membership, Minutes and rules governing Investigative Review Committee. Protocol on supervised work routines and the evidence to substantiate supervision Protocol for reporting and adverse event and the chain of command used. Protocol for hiring employees with licenses that are suspended retaining employees if their license has been suspended in a particular area Al emails and communications between Helene Varteles and Jerilyna Lamb- Pagone andor senior management concerning safety, working eonditons, patient complains or suspected abuse All emails and communications between the Department of Public Health tothe ‘Commissioner's Olice or senior management at CVE concerning any investigation, patient complaints, physician complaints, safety, or suspected patient abuse. {All emails between senior management and representatives of collective bargaining units ‘that represent Whiting Forensic employees conceming suspected abuse, working conditions and complains. ‘The number of patientlient transports between Whiting and Middlesex Hospital from 2011-Present [thank you in advance for supplying this information as it willbe vital to understanding wit led to the issues surrounding the management and conduct of Whiting employees, We must come to tems on where the system has filed in order to establish eorretive and preventative ation, Should you have any questons do not hesitate to contact me.
