We are college students of The National Teachers College taking up English2: Writing in
the Discipline.

As a requirement for our subject, we are conducting a survey about Factors That Affect
the Decision Making in Choosing College Courses of Grade 12 students of The National
Teachers College during the 1st Semester SY 2017-2018.

This study can help us discover the factors that affects the choices of Senior High School
students in taking college. Your honest and open participation is vital to our ability to accurately
discover the effects.

Enclosed with this letter is a brief questionnaire that asks a variety of questions about
your personal profile and current situations in your 12th Grade. We anticipate that this survey
should take no more than 10 minutes to complete. If you choose to participate in this survey,
your answers are completely confidential and will be released only as part of group summaries.
