I hereby confirmed that the above printed name is a student at the Faculty o{ Chemical
Engineering, Universiti Teknologi MARA Johor, Pasir Gudang Campus whom has fulfilled all
requirements to undertake industrial training program in semester November 2015 - April 2016.

The industrial training is part ol the requirement for the student to complete their Diploma in
Chemical Engineering course. The duration of the training is 15 weeks, starting from 29
November 2015 until 10 March 2016 (subject to change). We strongly believe with the
opportunity and guidance, the student will have sufficient exposure to the real working
environment. Therefore the student will have a chance to apply knowledge gained from diploma
study to real industrial applications.

Please be inform that the student will be covered with insurance by U|TM as stated in Pekeliling
Timbalan Naib Canselor (Bahagian Hal Ehwal Pelajar dan Alumni) Bil 1/2009.
