RADS is a term used to describe bronchial hyper-reactivity after exposure to an inhaled irritant. It
may also be known as irritant-induced asthma as described in a spectrum of work related asthma
entities.
RADS was first described in 1981 and later in more detail in 1985 after a review of ten subjects who
all presented with an acute onset of asthma-like symptoms after exposure to an inhaled chemical
irritant. It is primarily a clinical diagnosis that depends significantly upon a detailed and accurate
history (4).
One key to the diagnosis is the temporal relationship between an appropriate exposure and the
development of symptoms. Patients often present after a known massive exposure that often
prompts them to seek immediate medical attention. The initial injury in RADS is the inhaled irritant
causing direct damage to the bronchial mucosa with subsequent epithelial cell necrosis and
proliferation of the basement membrane. Bronchial biopsies later in the disease course reveal
desquamation of the bronchial epithelium with a chronic lymphocytic infiltrate without evidence of
mucus gland hypertrophy or smooth muscle hyperplasia (4).

In patients who may have been exposed to the offending agent from
occupational exposure, all efforts must be made to prevent re-exposure.
Patients should be urged to seek out alternative employment or different duties
with the same employer to minimize any risk of recurrence. If they remain
employed in the same setting of the original exposure, all respiratory
protective methods recommended at the worksite should be followed at all
times.

Urgencias por 3era vez en 10 das. El
ataque comenz sbitamente 6 hs antes.
Cinco das antes ella haba ido de alta con
Prednisona oral. Previamente utilizaba
salmeterol + fluticasona regularmente y
salbutamol segn necesidad. No fiebre,
hemoptisis ni tos productiva, aunque
rinorrea desde hace 3 das.

AEA: NN, Varn de 11 aos que consulta
por congestin nasal y rinorrea persistente
desde hace tres aos. Uso irregular de
fluticasona nasal y montelukast 5mg VO/d.
Hace dos meses comienza a sentir falta de
aire y tos ~ 20 minutos despus de iniciar
ejercicios lo que impide asistencia a la
escuela de futbol. No refiere sntomas
nocturnos .

beta-agonists for acute
asthma
165 trials were reviewed and eight were included; a total of 461 patients have been studied (229
with CBA; 232 with intermittent beta-agonists).
Current evidence supports the use of CBA in patients with severe acute asthma who
present to the emergency department to increase their pulmonary functions and
reduce hospitalisation. Moreover, CBA treatment appears to be safe and well
tolerated in patients who receive it.

This review has been updated in January 2008 and two new trials have been added.
2295 children and 614 adults are now included in 27 trials from emergency room and
community settings
Metered-dose inhalers with spacer produced outcomes that were at least equivalent
to nebuliser delivery. Spacers may have some advantages compared to nebulisers for
children with acute asthma.

Overall, this review did not identify a statistically significant difference between
these two methods for delivering bronchodilator therapy to children with acute
asthma or lower airways obstruction attacks. Care should be taken in the
interpretation and applicability of our results because of the small number of RCTs
along with few events available meeting the criteria for inclusion in the review,
absence of the primary outcome of interest and other clinically important outcomes in
the majority of included studies. The possible need for a face-mask in younger
children using home-made spacers should also be considered in practice.
