1. ELIGIBILITY: The Smilin Clydes Hot Dogs Hot Dog Eating Contest at the Childrens Museum in Oak Lawn is
open to the first 6 received applications. Contestants must be 18 years of age or older, responsible for ensuring
that they are in good health and medically able to participate, and not have any food allergies to eating hot dogs
or hot dog buns. Contestants acknowledge that this Contest is in good fun and agree to be good sports.

2. TO ENTER: READ CAREFULLY THESE OFFICIAL CONTEST RULES AND WAIVER OF LIABILITY ON THE
NEXT PAGE. Complete and sign this document. All entry forms become the property of Smilin Clydes Hot Dogs.

3. CONTEST RULES: There will be one competition for all qualifying contestants. Contestants will compete with
each other for a Grand Prize of $100. The time is subject to change without notice. Contestants must be present
at the Smilin Clydes Hot Dog Cart at 11:45 a.m. on Sunday, September 24, 2017 for final preparations. All
rulings by Judges of the event are final.

Each contestant will be given up to 15 hot dogs to eat as quickly as they can within 4 minutes. Contestants must
wait for the starting signal to begin eating and may not touch any hot dogs prior to the starting signal. Contestants
may eat either sitting or standing in their designated area, but may not leave their designated area before the
judges declare the winner. Contestants are allowed to use and drink water. Contestants must completely eat each
hot dog, including the bun, before picking up the next hot dog. Contestants must stop eating and promptly place
their hands down on the table or at their sides at the ending signal. Contestants may not put additional hot dogs or
portions of hot dogs in their mouth after the ending signal, but have 15 seconds to swallow any hot dogs already in
their mouths. If a contestant throws up or regurgitates during the Contest or within 2 minutes after the ending
signal, that contestant is disqualified. Judges will make sure these rules are followed, disqualify contestants who do
not follow these rules, and count the number of hot dogs eaten. The contestant to eat the most hot dogs in the
allotted time wins. Partially eaten hot dogs will not be counted unless there is a tie, in which case the consumed
portions of the hot dog will be rounded down to the nearest quarter dog. If there is a tie based on quarter portions
of hot dogs eaten, eat-offs will take place in one-minute increments, with a full hot dog count to include all fully
consumed quarter portions, until a winner is selected. Contestants must abide by any decision of the contest
officials and contest rules.

All contestants must present valid ID upon arrival for contest participation.
 Waiver of Liability/Media Release

Release before entering the Contest. Please read carefully.
In consideration of being allowed to participate in the Contest, I, the undersigned, for myself, my personal
representatives, assigns, heirs, next of kin, or anyone else entitled to act on my behalf, agree to the following:

I know that quickly eating large amounts of hot dogs is a potentially dangerous activity that involves
risk of damage to personal property, serious injury, or death, and I participate at my own risk. I
voluntarily assume all risks associated with the Contest including, but not limited to indigestion,
choking, vomiting, feeling nauseous or dizzy, hospitalization, forming a distaste for hot dogs, mental
trauma, contact with other contestants or hot dogs regurgitated by other contestants, and social and
economic losses which might result not only from my own actions, inactions, or negligence, but also
of others, the rules of play, the condition of the premises, or of any equipment used or food
consumed. Further, I acknowledge that there may be other risks not known or not reasonably
foreseeable at this time.
The Childrens Museum in Oak Lawn & Smilin Clydes Hot Dogs assume zero responsibility for my
participation in the contest. I am participating in the contest under my own will and assume all risks
associated with it.
I ASSUME ALL OF THESE RISKS and accept personal responsibility for all expenses, medical or
otherwise, that may result.
I PROMISE NOT TO SUE Smilin Clydes Hot Dogs, the Childrens Museum in Oak Lawn or any
other organizers and sponsors of the Contest, and the representatives and successors of any of
those entities (the Releasees). I WAIVE all legal claims that might result from my participation in
the Contest. The Releasees are not liable to me for acts, omissions, or negligence related to the
Contest.
If despite this waiver of liability, I or anyone on my behalf makes a claim against one of the Releasees
as a result of my involvement in the Contest, I will INDEMNIFY and hold harmless the Childrens
Museum in Oak Lawn, Smilin Clydes Hot Dogs or any officers, directors, board members, agents,
volunteers, and employees from liability for any claim, injury, expense, attorney fees, loss, or other
cost that might result from my participation in the Contest.
MEDIA RELEASE: I authorize the use of any pictures or videos taken of me during the Contest to be
used for publicity or any other purposes of the Childrens Museum in Oak Lawn and Smilin Clydes
Hot Dogs without any further consent or compensation.

I HAVE READ THIS WAIVER OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND
THAT I HAVE GIVEN UP SUBSTANTIAL LEGAL RIGHTS BY SIGNING IT. I HAVE SIGNED IT FREELY
AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A
COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT
PERMITTED BY LAW. IF ANY PORTION OF THIS WAIVER OF LIABILITY IS INVALID, THE REST
REMAINS VALID.
