Dear applicant,

In this folder, you will find the necessary documents you would require to complete the
filling up of your LPA Form 1:

2 You are encouraged to read through the Guides and Information Sheet before filling
up the Forms.

3 Before you submit the completed set of Forms to OPG via post, do refer to the
checklist to ensure that all necessary information and supporting documents are included.

4 After verification by OPG to ensure the application can be accepted, there will be a
mandatory six (6) weeks waiting period, after which your LPA will be registered if there are
no valid objections raised.
