A MARITIME DECLARATION

ISSUE: It is issued by the article 37 of the International Health Regulations

IS: It means that there isnt any illness on board and that all the crew are healthy or that in the
last five days there isnt any sick member of the vessel.

Also if there is any illness on board, the master has to report to the Health Protection Officer o
Autorithy Port where the vessel will arrive with the ADVANCE NOTICE OF ARRIVAL OR THE
NO CHANGE OF HEALTH STATUS FORM .

RECEIVE THE DOCUMENT: In practice, completed Maritime Declarations of Health are often
sent to the vessels agent for forwarding to health authorities. If there isnt any illness on board

RESPONSABLE: The Maritime Declaration of Health must be completed by the Master, in


addition this document is suported by the Second Mate or by the ships surgeon if there is
one, because he is in charge of the Medical essus.

anyone has died on board

anyone is sick

there is any case of disease which could be infectious
