ANNEX A OF WORK PROPOSAL

PROJECT : SEDA LIO PALAWAN

WORK PACKAGE : COMPLETE STRUCTURAL WORKS FOR
BLDG. 1, BLDG. 2, AND BLDG. 3 AND 4

This Bid Proposal Annex is hereby made part of the proposal and contract amount to
same extent as though it was originally included herein. This Annex must be attached
to the Proposal Form.

1. MATERIALS SUPPLIED BY THE VESCON & INSTALLED BY THE

CONTRACTOR (OFM) - In accordance with the construction schedule, the following
materials hereunder specified are guaranteed and fixed quantities to be supplied and
delivered by the VESCON to CONTRACTORs staging area at the Construction Site
or at any place designated by the CONTRACTOR and agreed to by the VESCON, for
CONTRACTORs installation.

Additional materials and quantities other than those specified hereunder which are
necessary to complete the WORKS in accordance with the bid plans and related Bid
and Contract documents shall be supplied by the CONTRACTOR at no cost to the
VESCON except those covered by Variation Orders. For additional works or
alterations ordered by the VESCON, the CONTRACTOR shall submit its proposal for
additional or deductive OFM with revised guaranteed quantity for VESCONs
approval.

Any unused OFM within 2.5% of the actual requirement of the WORKS, including
scrap, shall automatically belong to the VESCON. On the other hand, the unused
OFM delivered beyond or exceeding such 2.5% allowance shall either be charged to
or required to be purchased by the CONTRACTOR.

2. In case the CONTRACTOR's requested Guaranteed Fixed Quantity

(hereinafter referred to as, GFQ) of the OFM, if any, is less than the actual quantity
needed at site, the following conditions shall apply:

a. for QUANTIFIABLE and NON-QUANTIFIABLE items:

The CONTRACTOR must submit justification for the additional request of

items to the Project Manager
The additional items will be evaluated by VESCON to check if changes
are justifiable or not.

b. for NON-QUANTIFIABLE items:

If justifiable, the VESCON shall provide the additional items at no cost to

the CONTRACTOR, but the CONTRACTOR shall install these additional
items at no cost to the VESCON. If not justifiable, the supply and
installation of the additional items shall be provided by the
CONTRACTOR at no cost to the VESCON and is deemed included in the
Agreement Price.

c. for QUANTIFIABLE items:

If justifiable, the VESCON shall provide the additional items at no cost to

the CONTRACTOR. However, the CONTRACTOR shall install the
additional items with no extra charge to the VESCON.
 Any difference in quantities and material description of the OFM, which
are necessary to complete the WORKS based on the plans, equipment
schedule and related bid documents, shall be furnished by the VESCON
at no cost to the CONTRACTOR. However, CONTRACTOR shall install
these additional OFM without any additional cost to the VESCON

3. For all additional OFM covered by approved variation orders, CONTRACTOR

shall submit the adjusted GFQ for VESCONs approval.
