ACCIDENT

It is an undesired event that results harm to people, damage to property or loss to process.
It is usually a contact with a source of energy above the threshold limit of the body structure.

UNSAFE CONDITION: 10%

UNSAFE ACT: 90%

85-95% were caused by people making one or more critical errors while doing
something.

Critical errors

1. Mind not on task


2. Eyes not on task
3. Being in or getting in the line of fire
4. Losing balance, traction or grip

Critical errors were made in one or four more states

INCIDENT

It is an undesired event which is under slightly different circumstances, COULD HAVE


RESULTED to harm to people, damage to property, or loss to process. It is also called near
misses.

Lost Time Accident

The victim needs time off from work until he is completely healed and needs a
medical treatment and a support from a professional.

Medical Treatment Accident

The victim needs a medical treatment and a support from a professional.

First Aid Accident

The victim needs a medical treatment with minimum or no support from a


professional.

Near Miss/ Incident

It is a n event that could lead to 1 of the three previous cases.

To be safe we need eye, ear, head, hand, foot and respiratory protection.
