The majority of the world’s people currently live in rural and remote areas, representing a shift that is seen in your country. However, the majority of health workers and infrastructure remains in the cities and semi-urban areas. This concern is common to almost all countries and poses a major challenge to the timely and adequate providence of healthcare.

With the country’s increases in patient populations and lack of improvement in health status, health providers, health centers and governments are struggling to provide proper primary care and to keep patients out of emergency rooms, hospitals and other high cost settings. THE PROGRAM

Rijuven provides a 4 phase approach to implementing a successful CIB program. Once an infrastructural plan is created, care worker selection and training begins. As the program runs, Rijuven assists with public marketing of the program and provides outcomes based reporting and success stories.

Clinic In A Bag Setup and Care Coordination

1 Product Needs Assessment- First the number of CIBs per population, facility and/ or per care worker are determined based on the population served and underserved. Infrastructure Assessment- A hub and spoke design allows for physicians to remain centralized while new care workers service remote and rural areas.

Facility Needs- As CIB identifies more patients needing hospital care, additional monitoring devices are provided a hospital and health center beds.

Care Worker Selection and Training Physicians- A census is taken on the number of physicians by specialty are

2 available and their maximum patient load. Nurses, Midwives- Care workers already medically educated and experienced are trained to use CIB, how to share findings, and how to properly triage patients. Allied and Social Workers- Since CIB is easy to administer, and non-invasive for most diagnostics, care extension training is provided to non-medical workers in the most remote locations. Care Coordination- The hub and spoke care coordination model is tested in radiuses venturing further and father from clinics and health centers until an area is fully saturated with health screening opportunities. Telehealth- An international cohort of physicians is available for telehealth consultations when and where local physicians are not available.

TheNational Health Policy outlined the goals, structure, strategy, and policy direction of the health care delivery system in the country Roles and responsibilities of different tiers of government, including nongovernmental organizations, were clearly defined. The policy’s overall long-term goal is to provide adequate access to primary, secondary, and tertiary health care services for the entire Nigerian population through a functional referral system. The underlying principles and values of the Revised National Health Policy are as follows:

Health and access to quality and affordable health care are human rights.

Equity in health care for all citizens will be pursued as a goal.

Primary health care (PHC) will remain the basic philosophy and strategy for national

health development.

Good-quality health care will be assured through cost-effective interventions that are

targeted at priority health problems.

• A high level of efficiency and accountability will be maintained in the

development and management of the national health system.

• Effective partnerships and collaborations between various health sectors will be

pursued while safeguarding the identity of each.

The overall objective of the Revised National Health Policy is to strengthen the national health system such that it will be able to provide effective, efficient, quality, accessible, and affordable health services that will improve the health status of all citizens through achievement of the health-related Millennium Development Goals (MDGs). The main health policy targets that includes the following:

Reduce the under-5 mortality rate by two-thirds between 1990 and 2015

Reduce the maternal mortality rate by three-quarters between 1990 and 2015

Reduce the spread of HIV/AIDS by 2015

Reduce the burden of malaria and other major diseases by 2015

The national health policy identifies primary health care as the framework to achieve improved health for the population. PHC services include health education; adequate nutrition; safe water and sanitation; reproductive health, including family planning; immunization against five major infectiousdiseases; provision of essential drugs; and disease control. According to the policy, a comprehensive healthcare system delivered through PHC centers must incorporate maternal and child health care, including family planning services.

The national health sector is characterized by wide regional disparities in status, service delivery, and resource availability. In view of this situation, the government has initiated several interventions including the Midwives Service Scheme (MSS); the Subsidy Reinvestment and Empowerment Program, Maternal and Child Health (SURE-P-MCH); and systematic PHC infrastructure upgrades through the Ward Health System.
