1. initial = met the patient for the first time (data collection -> identity, patient history)

2. Reassessment / evaluation = formative evaluation and sumative evaluation

Element assessment

Data collection : identity, patient history

Data verification : do after doing data collection. Verify the data it is correct or not
Data organization : must be organize
Data interpretation : priority, the nursing problem
Data documentation : documentation the data with systematic

Types Data : there are subjective data (patient history) and objective data (hospital data)

After verify the data and collect the data, must be :

- Relevant with client needs

- Collected from variety source (ex: patient in psikiatric)
- Collected with appropriated technique (operational standar, prepare the tools)
- Collected in a systematic manner
- And document in usable format
