This agreement confirms the terms and conditions of your Nikita Uday Jain contract with Langley

Allied HealthCare and is subject to successful reference checks, proof of being granted a license
by the provincial College to practice and liability insurance .

1. Langley Allied HealthCare will take you as a Contractor as a Permanent part-time


Physiotherapist. Work will commence upon receipt of a license by appropriate provincial College.
Contract will stay in force until 6 months from date of joining and can be extended if needed.

2. Your hours of work and schedule will vary as per the appointments and your availability
.You may choose to work more than stipulated hours at your discretion .

3.You will be paid 50 percent split of the net billing amount once received to the clinic for the
co-relating sessions. The payments will be scheduled on a bi-weekly cycle. After 5 consecutive sessions
(non-promotional) you will be eligible for 55% split and after 10 sessions 60%. After each passing year
or 1440 hours (whichever occurs later ) your split will increase by 2.5% i.e. from 50 to 52.5% and so on
till a maximum of 60%.

4. You will be eligible for performance bonus at end of the year or after 1440 hours of
service (whichever occurs later) depending upon your inputs and expertise provided since after
joining the clinic.

5. Your work will comprise assessment, treatment, education and consultation services to clients
in our various programs and services, as well as administrative and committee work as required.
6. As conditions of your Contract , you must get and maintain registration with the appropriate
provincial college and liability insurance for safety of your own practice (As an independent contractor
this is absolutely essential). Documentation confirming the above must be provided to Langley Allied
HealthCare on an annual basis.

7. You agree to comply with the policies of the Contract as set out in its Companys Handbook and
Policies and Procedures Manual or as otherwise provided to you and as amended from time to time.
8. During the term of this Agreement and for Two (2) years thereafter, you shall not seek in any
way to persuade or entice any contractor or employee of Langley Allied HealthCare to leave or
terminate their contract with Langley Allied HealthCare. During the term of this agreement and for a
term of Two (2) year thereafter, you shall not solicit or induce anyone who was a Customer of Langley
Allied HealthCare in the Territory to transfer their business from Langley Allied HealthCare to any other
person. A Customer means any ministry, agency, organization, or individual with whom Langley Allied
HealthCare has conducted business with the organization prior to termination of this agreement.
Territory means a

10 kilometer (as the crow flies) radius from any office, including satellite offices or any office of
any of the Subcontractors, in the past 24 months.

9. Your Benefits will include health/dental and extended health care coverage and paid vacation.

10. During your Contract, you agree to comply with the provisions of the Personal
Information Protection Act, Human Rights Code and all other relevant and applicable legislation.

11. The contractor agrees to abide by the Colleges scope of practice as a health care
professional and do no such tasks that is not allowed by the college or the ethical codes of practice.

12. The practitioner will not share confidential and sensitive information about a patient or the
business to unauthorized parties in a way that harms the patient or the business.

13. Langley Allied HealthCare may terminate your Contract without notice or pay in lieu of failure
by you to comply with the provisions of this Agreement or for just cause. If the contract is terminated
for reasons other than what are previously stated ;any monies due from the organization to the
contractor would be paid in full as per the bi weekly payment cycle. If Contractor owes any monies to
the organization he or she is liable for full payment before the bi weekly payment cycle. If not paid,
amount due towards the Contractor will be withheld by the organization until the payment is provided
in full.

14. You may terminate your contract with Langley Allied HealthCare by providing us with a minimum of

21 days of written notice.

If these terms are agreeable to you, please sign below and return this letter to us. Please make a copy of
this letter for your records.

15. A rent of $100 per month will be charged to the contractor by the firm for using the
facilities provided.
