Summary
1.1 Introduction
The Contract ER356QP is for the provision of Qualified Person(Supervision)Services
for the main contract ER356 - which was awarded to Koon Construction & Transport
Co. Pte. Ltd. in July 10 . ER356QP services commenced on 26 th October 2010 and due
to complete by 30th June 2012, to be followed by a 12 months servicing of the
contractors defect liability period.

The ER356QP Contract reference is as follows:

1.2 Status of Main Contractor works


a) The Contractor has carried out the following works in June 2012:--

i. Casting for Diaphragm Beam & slab, parapet wall


ii. The Contractor carried out Deck Slab.
iii. The Contractor Carried out Road Pavement & Base Slab Retaining wall.

QP(S) Works
a) QP(S) -C&V Engineering PTE. LTD. Commence QP service on 26 th October 2010.

b) QP(S) supervised /observed/reported on the following : --

i. Inspection for: - Casting of Crash Cushion Concrete Pad.


ii. Casting of Parapet Wall.
iii. Review Method of statements/Test Reports submitted by Contractor.

2.0 Scope of Main Contract and QP(S) Works


2.1 Main Contract Works
The Main Contract ER356 had been awarded to Koon Construction & Transport Co. Pte Ltd
in July 10.

The works comprised works such as:

Design and construction of a elevated vehicular RC deck structure over


open drain for the ingress from TPE to Changi North Industrial Park

Design & construction of a Structure over canal, a signalized traffic junction, an


acceleration lane & at grade roads for egress from Changi North Industrial Park into
PIE.

Modification & Recambering of existing carriageway to tie in with the levels of the
adjacent connecting roads.

Widening / recambering / raising / milling down and overlaying of existing


carriageways, construction of rigid pavement at at-grade road
intersections and accesses to adjoining developments as shown in the
Drawings.

Design, construction & modification of earth retaining structures including


Retaining walls, slopes & embankments, etc.

All drainage works (roadside drains, culverts, sumps, etc) including all ancillary
works but not limited to the complete design, obtaining necessary approval from
relevant authorities including but not limited to PUB(Drainage), supply of materials
and equipment, construction and handing over to relevant authorities;

All sewer diversion works including but not limited to the complete design, obtaining
necessary approval from relevant authorities including but not limited PUB
(Sewerage), supply of materials, construction and handing over to relevant
authorities;

Resiting / reconstruction of existing signage, guardrails, railings, bollards and all


others affected by the Works

Site clearance, earth works, turfing, and construction of footpath, trench


works and trees planting.

Miscellaneous works.

All temporary and monitoring works related to the projects.

2.2 QP(S) Works


The Contract ER356QP is for the provision of Qualified Person(Supervision) Services for
the main contract ER356 - The scope of works is shown in Appendix 1A - QP(S) scope
of works.

QP(S) services include the provision of professional engineering supervision services as


required under the Authoritys requirements and /or the requirements of other authorities
and agencies and to comprise:

Provide qualified site supervision staff as required under the Building Control Act to
support the QP(S);

Ensure that the construction works are carried out in a safe manner in
accordance with best construction practices; and

Any other professional engineering services as set out in the scope of Services.

3.0 Transmittals

3.1 Letters to LTA on Main contractors matter


The list of letters to LTA related to Main contractor is as shown in Appendix 2A (Review of
Main Contractors submissions).

3.1.1 Statistics on Letter Review Time


According to the Quality Performance Evaluation Criteria as stated in C&V`s Project Quality
Plan (PQP section 2.5) 50% of all letters must be reviewed within 7 days and 55% after 1 years. It
also states that 90% of all letters must be reviewed within 14 days and 96% after 1 year. Table 3.1.1a
shows the letters reviewed statistics for the month of August 2011 whereas Table 3.1.1b shows the
cumulative statistics of all letters received and reviewed since the beginning of the project. As can
be seen, all targets set had been achieved.

3.2 Letters to LTA on matters other than Main Contractors issue

The list of letters to LTA on issues other than those linked to Main contractor s
submission are as shown in Appendix 3A (Letters to LTA , except on Main Contractors
submission/ issues ).

4.0 Construction Drawings

5.0 Meetings, inspections and coordination

The list of meetings held in this month is as shown in Appendix 5A (list of Meetings held
/attended).

5.2 Inspections carried out


Inspections carried out for this month are summarised in Appendix 5B (List of Inspections).

6.0 Main contractor construction works, safety and monitoring and


areas of concern

6.1 Main contractor construction works


a) Casting of Deckslab, Beams, Diaphragm wall, Top Slab & Paraphet Wall

6.2 Safety and monitoring work


a) All safety issues were closely monitored. Any potential safety problems that were
noted on site were immediately conveyed to KCTC for rectification. Items noted so far
were of relatively minor in nature & was rectified by Contractor within the given time
frame.

b) No SQNs were issued for the month of June 2012.


c) The Contractor conducted their 18th monthly safety committee meeting on 29th March
2012 in our presence, regarding the safety precautions & awareness of workers. Safety
Toolbox meetings were held on a daily basis.

6.3 Area of concern


a) SAFETY REVIEW - We have frequently reminded KCTC that safety matters are always
top of concerns. So far 1 minor work related accident was reported.

b) INTERNAL PROCEDURES - The Contractor has been reminded to ensure that Method
Statements are submitted at least 28 days before commencement of construction
activities to give sufficient time for the Method Statement to be reviewed. This is
because there are always comments from the LTA and QP(S) which needs to be
addressed before the Contractor is allowed to commence works.

c) UNAUTHORISED WORKS None to report for the month of June 2012.

So far the full force of 2 RTOS have been deployed (since 14 th Feb 11), please see Table 8.1

APPENDIX 1A - QP(S) Scope of Works


QP(S) is to provide the required Qualified Person (Supervision) [QP(S)] to

a) provide professional engineering supervision services as required under the Authoritys


requirements and /or the requirements of other authorities and agencies;

b) provide qualified site supervision staff as required under the Building Control Act to
support the QP(S);

c) ensure that the construction works is carried out in a safe manner in accordance with
best construction practices; and

d) Any other professional engineering services as set out in the scope of Services.

QP(S)s primarily role is to ensure safety and quality construction by the main contractor and
serve as the technical advisor to LTA on the matter of supervision . As no formal contract exists
between The Main contractor and QP(S) , all dealing are through LTA and no changes in the design
is to be accepted during the supervision stage , unless the QP(Design ) and LTA have approved it.

As the main contract is a Design and Build contract , the main contractor has engaged the design
firm (Fong Consult Pte. Ltd) to undertake the full design based on LTAs tender development
drawings and also based on further data/information collected by the main contractor.

The main contractor had also engaged Er. Dr. Goh Sew Khee as PE for temporary works as well as
PE (TERS) for temporary earth retaining structures .

At the same time , LTA has also engaged Accredited Checker ( AC) namely Mr. Chitaldroog
Keshava Murthy to provide the required check on permanent design of the works, as well as for
the checking of the AIP documents. Where necessary, an AC will also be engaged by LTA.

QP(S) role is mainly to carry out supervision duties and checking services for method statements
and other proposals submitted by KCTC. The temporary works which are not deemed to be checked
by an AC are also to be checked by QP(S). However, the permanent design check , obtainment of
the permit to carry out structural works from the BCA, and modifications, if any, at construction
stage are to be handled by the QP(D) .
