Project Description

The purpose of my grant proposal is to provide the community of Buckingham County

the health awareness of benefits provided by an annual Health Fair Event. Therefore, the kind of

ideas for this grant are in the area of community health which focuses on nurses promoting

health and wholeness for the community (Nurses for Good, 2015). As a result, the residents

would come together and participate in various health care screenings to rule out health

problems. This will be, especially, helpful for members of the community who do not practice

preventive health and for those who are not proactive when it comes to living a healthy lifestyle.

This group would include those who do not have a physician and do not utilize the local health

department or the community health care clinic. Numerous health screenings, such as, blood

pressure reading, glucose level, cholesterol level, and INR would be provided in order to assess

for various underlying health conditions. Also, nutritional education with weight, height, and

BMI could be provided in an attempt to avoid health concerns that are a direct impact of obesity.

The Health Fair event would, hopefully, bring awareness, education, and provide ways to treat or

prevent illnesses in order to help promote better health and prevent further complications in the

community. Finally, the health care professionals would provide counseling to each individual

in order to encourage follow-up and referral to a primary care physician.

Statement of Need

The need that is being met is on a community level in which nurses promote health

and wholeness in the community (Nurses for Good Grant, 2015, p. 5). The idea is to provide

free health care screenings, share important health education, and provide appropriate referrals to

health care professionals once the screening is complete to those individuals who are not actively

involved in preventive or maintenance healthcare. The population that will be served if this

proposal is implemented will be the individuals in the county of Buckingham, Virginia who

otherwise do not have healthcare. The ages may range from 18 years to older. If the event goes

well, younger patients will be included. The primary purpose of the health fair is to promote

health and welfare to those who are without medical resources.

Proposal Details

The activities to accomplish this project will include setting up various venders at

different health care stations in order to measure which area of concern is recognized. In order to

accomplish this, the following objectives will be addressed:

Assessment of various health care conditions through health screenings.

Promotion of positive health and well-being through education and counseling

related to the benefits available to provide permanent change towards a positive

approach.

Referrals to appropriate professional organizations in order to treat or prevent

health care issues.

The following health care screenings will include:

Blood pressure reading

Cholesterol level

Glucose level

INR

Visual exam

Weight/Height/BMI

Influenza and pneumonia vaccine

Education and Referrals

The plan to have eight stations set up in order to accomplish the above will be initiated

three months in advance. The personnel will include 10 medical professionals, 7

individuals who have medical/clerical/security background who attend the church and

can assist with set up, clean up, parking, educational material/pamphlets, and advertising

in the local newspaper and church bulletins in the surrounding areas. The event will take

place in the social hall at First Baptist Church, 16980 Oak Street, Dillwyn, Virginia

23936. The event will be held in the fall of 2016 and will take place annually.

Advertisement in local newspaper and flyers done by 3 clerical staff at church

Location/Parking performed by 2 security officers at church

Registration done by 2 Clerical Staff from church

8 different stations done by 8 medical professionals

2 additional medical staff provided to assist with breaks

Detailed Spending Plan

Salary and Wages: Estimated at $2,160.00. However, this will be an annual event

that would be staffed by professionals on a volunteer basis. 10 medical professionals

at $20.00/hr. for 8 hours, 7 additional personnel at 10.00/hr. for 8 hours.

Benefits and Payroll Taxes: as stated above, this is a benefit health fair that will be

based, strictly, on volunteers. Therefore, no benefits or payroll taxes will be involved.

However, it is recommended that a donation be made to the church for providing the

location and supplies needed in order to hold this event. Also, the personal

gratification of doing good for those in need, as nurses should do, in order to follow

the goals of Bon Secours Strategic Quality Plan to faithfully respond to Gods gift of

compassion, healing, and liberation in which we commit to bring our communities to

wholeness, ensure our care is extraordinary transform our health delivery, express our

catholic identity, and to liberate our peoples potentials (Strategic Quality Plan, 2013-

2015, p. 1).

Consultants and Professional Services: My church has an offering that provides for

situations that arise, such as this. Also, I have enough friends and family who are in

the medical profession who would be happy to assist in order for the plan to be

successful. The ladies in the church, who are not medically oriented, will help with

other details that do not require a medical background. There are four registered

nurses in my family, alone, and one FNP who would help plan the health fair and

actively participate. Additionally, a physician that is employed by the clinic is a

personal friend and participates in activities, such as this, in order to benefit those in

need and who do not have a PCP. This would be a great place to start.

Travel: Travel expenses should be minimal because Buckingham County and the

surrounding towns, are small in area. The active participants live in the county and

the patients would be responsible for their transportation to and from the health fair.

However, if the participants need transportation, a church member could provide

transportation with the church van. $500.00 allowance for gas.

Training and Education: The medical personnel who volunteer to participate will

already be trained.

Equipment: Glucometers & supplies, alcohol wipes, 2 x 2s, bandage tape, BP cuffs,

scales with measurement stand, Snellen chart for vision, tuning fork for hearing, flu

shots provided by CVS to FNP for administration. $1,500.00.

Depreciation: non-applicable.

Office materials and supplies: Computer, copy paper, printer, notebook, register

book, pens, pamphlets. $1,500.00

Printing: included in the above except for advertisement in the local newspaper which

is $150.00.

Utilities: The event would be located in the church and the utility cost would be

included with the function.

Postage and Mailing: There should be no need for postage or mailing because, other

than being advertised in the local newspaper, advertisement and communication will

be done through e-mail and church bulletins.

Rent and Occupancy: Rent and occupancy will be free of charge. This event will be

held in our local church which has a fellowship hall that can accommodate 250

people.

Marketing: Farmville Herald (local newspaper), WFLO (local radio station), flyers,

pamphlets, and through the church bulletin. $1000.00

Indirect expenses: expenses for refreshments served following the event or for those

who have a low glucose level or are dehydrated. Also, drawings will be held for

prizes to encourage participation. $3,000.00

Other expenses: A donation to the church is recommended in appreciation for the

location, utilities, and participation by church members. $10,000.00

Total: $19,810.00.
