Document-Return Service using a ASC Location

Applicants must prove their identify using one of the following methods:
1. Applicants retrieving their personal documents must prove their identity with any of the following forms of documentation:
a. Official or State issued Identification card (with photo)
b. Official drivers license (with photo)
c. Birth Certificate
2. Parents retrieving documents for their children must provide proof of their relationship by presenting the following documents:
a. A copy of the childs birth certificate or adoption decree, with at least one of the parents full legal name displayed on the
document.
b. Any of the parents must prove their identity by presenting a photocopy of official Identity Document issued by the state (with
photo), his/her name must match with the certificate of birth or child resolution adoption.
3. Adult applicants may authorize a third party to retrieve their documents. The third party must present the following documents:
a. A notarized letter of authorization signed by the applicant that contains the third partys full legal name.
b. A photo copy of one of the applicants identification documents listed in item #1.
c. The third party must also submit a photocopy of an official identity document issued by the Estate (with photo). The name on the
ID must match with the name on the letter of authorization.
Any documents not retrieved within 30 days will be returned to the originating Consular Section.
