Heart Failure (HF)

Heart failure patients with documentation in the hospital record that left
I-HF-2 ventricular systolic (LVS) function was evaluated before arrival, during
hospitalization, or is planned for after discharge

Psychiatric patients who were placed in physical restraints during their


I-HBIPS- inpatient hospitalization. This measure will determine the total number of hours
2 that patients were maintained in physical restraints for those admitted to a
hospital-based inpatient psychiatric setting

Psychiatric patients who were placed in seclusion during their inpatient


I-HBIPS- hospitalization. This measure will determine the total number of hours that all
3 patients were maintained in seclusion for those admitted to a hospital-based
inpatient psychiatric setting.

Nursing-Sensitive Care (NSC)

Patients that have hospital-acquired (nosocomial) pressure ulcer(s)


I-NSC-2 (category/stage II) on the day of the prevalence study.
Note: Please see Appendix E for details on how to collect this measure.

All documented falls with or without injury, experienced by patients in a


I-NSC-4
calendar month.

I-NSC-5 All documented falls by a patient with an injury level of minor (2) or greater.
