3. Nature of Event:
Boiler hand tripped due to main steam equalizing lines drain MS-109 was having heavy leakage.

4. Name of Protection Operated (First Up) and any other main protections Operated:
I) Gen. reverse power protection trip & Low forward protection alarm.
II) 32G1x/32G2x & 37G1x/37G2x.
III) STG2 TR REL 186 B operated & STG2 TR REL 286 B operated. Tripping sequence logic is ok.

5. Observations: -
Main Steam equalizing lines drain MS-109 was having heavy leakage.

6. Remedial Action / Work done:

Main Steam equalizing lines drain MS-109 was found leakage at its stub joint. Welding at stub joint of equalizing drain line is
done. Boiler purged out and lighted up at 12:30 Hrs on dated 13/12/2015 and TA set was synchronized at 16:58 Hrs on
dated 13/12/2015.

7. Root Cause Analysis :-

Main Steam equalizing lines drain MS-109 was found heavy leakage due to erection welding joint defect.

8. Preventive action suggested (Short Term):- NIL

9. Preventive action suggested ( Long Term) :-

High pressure drain lines welding joints inspection will be carried out in forthcoming AOH.
