JOB DESCRIPTION

Post Title: Crisis Resolution/Home Treatment Team Mental


Health Worker

Grade: Band 3

Responsible to: Crisis Team Manager

Accountable to: Crisis Team Manager

Key Relationships: Crisis Resolution Home Treatment Team Staff, In-


patient Unit, Assessment and Recovery teams, Duty
Workers , Psychiatric Liaison, Day services, User
Groups,
Non-statutory agencies.

Job Purpose
To work under the guidance of qualified staff to provide Crisis Resolution/Home
Treatment (CRHT) as an alternative to hospital admission to patients experiencing a
mental health crisis.

Main Duties and Responsibilities

To work intensively with clients who are experiencing a mental health crisis.
The CRHT Support Worker will work closely with the team to deliver the CPA/
Care Management Plan. This may often include encouraging the clients self
care through a planned programme of personal, practical and emotional
support and participation in domestic tasks and monitor concordance with
treatment.

To accompany and escort clients as necessary and desirable for their well
being, as part of the Care Plan.

To carry out vital observations as appropriate and report accordingly.

To participate in the assessment of referred patients.

To conduct risk assessment under supervision.

To conduct ongoing mental health risk assessment and reviews.

To keep regular records and contribute to multi-agency / MDT discussions.

To prepare discharge summaries to facilitate sharing of information and


seamless patient care.
To ensure working areas, including clinical areas, are kept clean and tidy with
the aim of achieving a clean environment for service users, staff and the
public. To be familiar with and adhere to Trust infection prevention and control
policies.

To support clients with benefits.

In a multi-agency context, to liaise and refer for services as needed.

To liaise closely and work with the multi-disciplinary team in the planning,
implementation and review of care packages for patients admitted to Home
Treatment.

To take part in individual and group supervision, staff meetings and training as
required.

To bring to the attention of the team and if necessary the line manager any
issues of concern in relation to clients welfare.

To work a 37.5 hour week over a 24 hour shift pattern.

To carry out ward screening with support.

To participate in CPA and other relevant meetings with patients

To carry out other duties and responsibilities of a similar nature to those


described above necessary to meet the needs of patients and the developing
service.

Infection Control
The Trust is committed to the prevention and control of infection and
operates an infection control policy which is informed by the Health and
Social Care Act 2008. It is the responsibility of all employees to be
aware of the infection control policy and procedures and the importance
of protecting themselves, service users and visitors.
