Dear Mr. Fuller

This project has been inspected for acceptance. Before this project can be accepted, the following items need to be
completed. Repairs are now needed for the following locations:

Janet SW34th to Western.

1. Cracks all along the entire project need to be sealed.
2. Overlay is peeling away from existing surface in front of 3501 Janet.

Hillside: Bell to 1-40.

1. Pot-holes at the pavement tie at Bell and Hillside.
2. Pot-holes forming inWBL (west bound lane) at Auto Glass Unlimited on the white stripe.
3. Base failure in WBL at Domino's Pizza.
4. Pot-holes and base failure in WBL just west of Rutgers/Radiant.
5. Pavement failure WBL in front of 5800 Hillside.
6. Pavement is failing in WBL at Cornell.
7. The manhole and the pavement around the manhole inthe EBL (east bound lane) has sunk and
both need to be adjusted to the level of the new driving surface in front of 5404 Hillside.
8. Pot-hole forming in the EBL at 5408 Hillside.
9. Pavement is raveling along the N-S construction joint at 5404 Hillside.
10. Pot -hole (clay ball) WBL at 5320 Hillside.
11. Pot -hole (clay ball) WBL at 5304 Hillside.
12. Pot-holes WBL at 5302 Hillside.
13. The manhole and the pavement around the manhole at 5300 Hillside has sunk and both need to be
adjusted to the new driving surface.
14. Pot-hole WBL at 5210 Hillside.
15. Pavement failure in the EBL at 5306 Hillside.
16. Pot-hole EBL along the toe of gutter at 5306 Hillside.
17. Several streets going south off ofHillside thepaving tie hasa drop down tothenew overlay.
18. The manhole and the pavement around the manhole at Star Lane has sunk and both need to
be adjusted to the new driving surface.
19. Pot-hole WBL at 5024 Hillside.
20. Pavement is raveling and has failed WBL at 5018 Hillside.
21. Pot-holes WBL at 5010 Hillside.
22. Remove excess junk HMAC from the flowline of gutter at 4824 Hillside.

 23 The paving tie WBL at the access road at station 38+50 +/- is a jump up from the existing
pavement.
24. The pavement tie in the WBL at the 1-27 underpass has failed.
25. Cracks along the entire project.

SE 3rd : Arthur to Grand.

1. Pavement failures at 3R0 and Pittsburg
2. Cracks along the entire project.
3. Pavement in the EBL at the SE 3'" underpass has reveled and failed
4. Remove and replace the overlay EBL, inside lane from the west side of Ross to the west 100'

Georgia: I-40 to SW34th.

1 All along the west side of Georgia the approaches, alley aprons, sidewalks and the flowline of the
curb and gutter need to be swept.
2. Water valve box in the SBL just north of Duniven needs to be adjusted to the new driving surface.
3. Seal cracks entire length of Georgia

Bell: l-40toSW11th.
1 Construction debris on walks, approaches, alley aprons and in the flow line of gutter along the east
side of Bell between I-40 and Lawrence.
2. Pavement failure NBL at Lawrence and Bell.
3. Pavement failure in the SBL at Westwind
4 Construction Debris on walks, approaches, alley aprons and flow line of gutter along the west side of
Bell between SW11th and I-40
5 Pot hole in the SBL at Westgate
6. Cracks along the entire length of Bell
