Use this template to document a risk assessment to manage health and safety hazards and risks.

Activity Description: In this activity students will walk to the park. students will work in the park to complete there
activities set by the Teacher. Students will move around the park identifying features of the places and also
taking pictures and drawing. students will be accompanied by an adult throughout the day. the groups ratio will
be one adult to five students.

Animals/ Insects Low Students will be in the park where people walk past with animals.
Students will be under constant Supervision. Students will be
working in the park around gardens so there is a chance there could
be insects around the garden. Students will be able to wear insect
repellent if their parents agree to it in the consent form. Students will
be encouraged to stay away from the gardens where possible.
Driveways / Pathways/ Low There will be a lot of traffic around the park as the park is near the
Vehicles highway. Students will be in groups of five and the ratio of adult to
students will be one to five. Adults will need to ensure that students
are being supervised at all times. Each student will need to stay in
their group with the adult. Students will be required to move around
with the group and ask their adult before moving to look at
something else in the park.
Play Ground Low After the trip through the museum students will have the
opportunity to play on the playground in the park. Students
will be supervised by the teacher and parents and guardians.
Students will need to be playing safely and respectfully on
the playground.
Other details:
Students will need to wash their hands before eating their lunch and after using the bathrooms.
students will be encourages to use their manners when communication with others. A first aid kit
will need to be taken and leftwith the groups. all groups will need to be within eye contact of each
other.

Details:The control measures have been used to ensure all students have an enjoyable time along with
being safe. The measures are realistic and are designed on the chances of something happening
during the course of the day. There may be need for additional measures to be included as the field trip
occurs. This will be done by the teacher and all adults and helpers will be notified of the additions or
adjustments to the measures.
